摘要 心脏再同步治疗(CRT)应用于临床至今已有20多年,已被公认为是治疗伴有束支阻滞心力衰竭(心衰)患者最有效的方法[1].传统的CRT是在右心房、右心室双心腔起搏基础上增加一个左心室位点的起搏,即“三腔起搏”,以达到治疗心衰的目的.对于中-重度心衰、QRS时限增宽、射血分数降低的患者,CRT在不增加心肌耗氧量前提下增加每搏量,提高心脏做功效率,使心室收缩非同步的心衰患者心脏再同步,明显改善患者的长期生存质量,逆转心脏重构,降低心衰患者的入院率及病死率[2].然而,传统的CRT中仍有30%左右的患者无反应,后者主要认为与存在心肌瘢痕、不均一性的电激动、非最佳起搏位置以及应用单左心室导线不足以实现左心室同步协调运动等相关[3-5].为了提高CRT的反应率,近年CRT领域提出了多点起搏(multisite pacing,MSP)的概念.本文就多点起搏的研究进展做一综述.
