摘要 目的探讨缺血修饰白蛋白(IMA)、NT-proBNP和高敏肌钙蛋白I(cTnI)三者及联合检测在急性心肌缺血早期诊断中的临床应用价值。方法收集急性冠脉综合征患者作为病例组及健康者作为对照组各120例,采用白蛋白钴结合试验、IMA试剂盒检测血清IMA;采用ROCHE E170免疫电化学发光分析仪检测NT-proBNP;采用心梗定量分析仪检测cTnI,并对结果进行敏感度和特异性分析。结果病例组IMA、NT-proBNP及cTnI的水平明显高于对照组(P<0.05);三者联合检测的敏感度和特异性都优于单个指标的检测。结论 IMA、NT-proBNP、cTnI三者的联合检测,可能是心肌缺血早期的一个有效的生化标志物组合,能作为临床早期可靠诊断心肌缺血的辅助手段。 Objective To investigate the early diagnostic value of combined detection of ischemia-modified albumin( IMA),NT-proBNP and Troponin I( cTnI) for myocardial ischemia. Methods A total of 120 patients with acute cardiac syndrome and 120 healthy controls were enrolled. we detected IMA levels by albumin cobalt binding( ACB) test,NT-proBNP by electrochemiluminescence on ROCHE E170 instrument,and cTnI by the quantitative analyzer. Results IMA,NT-proBNP and cTnI levels were significantly higher than that in healthy controls( P 〈 0. 05). The combined detection sensitivity and specificity were higher than to single index detection. Conclusion IMA,NT-proBNP and cTnI joint detection for early myocardial ischemia may be a useful biochemical marker combination,which can be used as a means for reliable early diagnosis of myocardial ischemia.
