消化内镜隧道技术在食管疾病中的应用进展

消化内镜隧道技术是近年来出现的一种内镜微创治疗新技术,其通过建立黏膜下隧道在隧道内外进行肿瘤切除,手术创伤小、安全性高,已广泛应用于消化道疾病的诊治。食管是一纵行的肌性管道,其黏膜下层由疏松结缔组织组成,肌层相对发达,这种结构特点使得隧道技术在食管疾病的诊治中独具优势。本文通过对内镜隧道技术的产生及发展进行阐述;对其在食管黏膜层、黏膜下层、固有肌层疾病及食管腔外疾病中的应用进行详细论述;对内镜隧道技术的未来发展进行展望,以期增进临床医师对该技术的认识和学习,促进其临床应用,使其更加成熟,更好地服务于患者。

Digestive endoscopic tunnel technique is a minimally invasive endoscopic treatment technology newly emerging in recent years, which can remove the tumors inside or outside the tunnel with small tissue damage and high safety by establishing subnmcosal tunnel, and has been widely applied in diagnosis and treatment of digestive tract diseases. Esophagus is a vertical muscular tube with loop connective tissue and relatively well developed muscular layer. These structural characteristics make the tunnel technique as unique advantages in the diagnosis and treatment of esophageal diseases. In this paper, the produce and development process of endoscopic tunnel technique are described in detail, and its application in the esophageal mucosa disease, submucosal and muscularis disease, and the disease outside esophageal cavity are discussed respectively. Finally, the futural development of the endoscopic tunnel technique was prospected. We aimed to help the clinician understand and learn more about this technique, thus promoting its clinical application, making it more matural, and suppling better services for patients.
