摘要 目的基于CBCT制作的个性化种植外科导板又称为CBCT导航种植模板,对于种植体的准确植入有重要意义。目的是制作一种简易CBCT导板,并评价其准确性。方法翻制下颌摸型。特制一个塑料导板,上嵌入4个金属珠作为放射标志物并将塑料导板放在下颌摸型的咬合面上。对带外科导板的模型进行CBCT照相,在CBCT图像上用虚拟植体进行模拟种植,获得虚拟植体相对于导板的三维空间位置数据。采用特制的5轴线钻孔机根据位置数据在外科导板上制备出种植引导孔。将制备好的外科导板放置在下颌上再次进行CBCT照相。对第二次成像的种植引导孔的中轴线与第一次成像的虚拟植体轴向间的差异进行评价。结果入点的误差和中轴的角度差异分别为(0.47±0.27)mm、(0.57±0.23)mm和(0.64±0.16)°、(0.57±0.15)°。结论本研究中的CBCT引导种植摸板准确性高。每个位点的20度入点误差比10度的入点误差大。 Objective Personalized implant surgical guide based on CBCT is also called CBCT navigation implant template,having important significance upon accurate placement of implants. The objective of this study is to make a simple CBCT guide and evaluate its accuracy. Methods A mandibular model was reproduced and a plastic guide plate was hand-tailored. The guide plate was embedded with 4 metal beads as radiation markers and placed at the occlusal surface of the mandibular model. The model with surgical guide was photographed using CBCT. Simulated implanting was processed on the CBCT images using virtual implants,so three-dimensional space position data of virtual implants with respect to guide plate could be collected. The implanting guide holes were manufactured on the surgical guide plate according to the position data using specifically designed 5 axis drilling machine. The well-prepared surgical guide was placed on the lower jaw and photographed by CBCT again. The axial difference between axle wires of implanting guide holes in second imaging and axes of virtual implants in first imaging was evaluated.Results The errorof enter-point and the difference of axial angle were( 0. 47±0. 27) mm,( 0. 57±0. 23) mm and( 0. 64±0. 16) °,( 0. 57± 0. 15) °,respectively. Conclusion In this study,the accuracy of CBCT guided implanting template was high and the error of 20 degree enter-point was larger than 10 degree enter-point at each locus.
