Bakri球囊在介入治疗凶险型前置胎盘中的应用

Application of Intrauterine Packing with Bakri Balloon in Interventional Treatment of Pernicious Placenta Previa-induced Postpartum Hemorrhage

目的:比较宫腔Bakri球囊填塞序贯介入镶嵌式治疗(SITHT)与宫腔纱条填塞SITHT对凶险型前置胎盘(PPP)产后出血的防治效果。方法:选取在我院诊治的72例PPP产后出血患者,其中16例给予宫腔纱条填塞SITHT(对照组),56例给予宫腔Bakri球囊填塞SITHT(观察组)。比较分析2组患者的宫腔填塞时间、剖宫产术中出血量、子宫切除率、剖宫产手术时间、剖宫产术后24 h出血量等指标。结果:观察组的宫腔填塞时间、剖宫产术中出血量、剖宫产手术时间、剖宫产术后24 h出血量较对照组少(P<0.05或P<0.01);2组子宫切除率、术后感染发生率、术后抗生素使用时间、手术费用、治疗费用比较均无显著性差异(P>0.05)。结论:SITHT法防治PPP产后出血术中宫腔Bakri球囊填塞较宫腔纱条填塞操作简单、止血快、止血疗效肯定、安全,具有临床推广应用价值。

Objective：To compare the effect of intrauterine packing with Bakri balloon in sequential interventional treatment of hybrid therapy （ SITHT） and the intrauterine ribbon gauze in SITHT for postpartum hemorrhage induced by pernicious placenta previa （ PPP）.Methods：A total of 72 PPP women with postpartum hemorrhage were included in this study .Of them, 16 cases receiving intrauterine packing with ribbon gauze in SITHE were enrolled as control group , and 56 ones receiving intrauterine packing with Bakri balloon in SITHT were enrolled as observation group .Indexes such as intrauterine packing time ,intraoperative blood loss , hysterectomy rate , operation time , 24-hour blood loss after hysterectomy were analyzed and compared between the two groups.Results： The intrauterine packing time , intraoperative blood loss , operation time and 24-hour blood loss after hysterectomy decreased significantly in the observation group than those in the control group （ P 〈0.05 or P 〈0.01 ） .No significant differences in hysterectomy rate , incidence of postoperative infection , time of postoperative antibiotic usage , surgery cost and treatment cost were found between the two groups （P＆gt;0.05）.Conclusion： Intrauterine packing with Bakri balloon in SITHT is a simple, safe and effective procedure for controlling postpartum hemorrhage in PPP women .
