耐多药肺结核的监测分析

Surveillance and analysis of multi-drug resistant tuberculosis

目的分析耐多药肺结核患者的监测、治疗情况,为制定相关措施提供参考。方法通过痰培养和药物敏感性检测,在耐多药肺结核可疑患者中发现耐多药肺结核患者,对纳入患者进行为期2年的规范治疗管理,收集调查数据,分析患者治疗情况。结果共计检测1 046例可疑患者,在获得药敏结果的592例肺结核患者中,发现耐多药肺结核82例,耐多药率为13.9%;不同类别肺结核患者间发现的耐多药差异具有统计学意义(χ2=106.3,P<0.001);复发患者中发现的耐多药患者最多,占51.2%。共计纳入治疗53例,患者治疗成功率达57.1%。结论复发患者是耐多药筛查发现的重点对象,对耐多药患者实施全程的规范化治疗管理是获得治疗成功率的保证。

Objective To analyze the result of detection and treatment of multidrug- resistant tuberculosis so as to provide evidence for the action in Guizhou. Methods Multidrug- resistant tuberculosis patients were found by sputum culture and drug susceptibility test（ DST） in multidrug- resistant tuberculosis suspects; two years＇ standard therapy was carried out on patients,and the result of the treatment was analyzed. Results 1 046 suspicious patients were tested,there were 82 multidrug- resistant tuberculosis patients in 592 pulmonary tuberculosis patients,the multi- drug resistance rate was 13. 9%,the multidrug- resistant differences in different pulmonary tuberculosis patients were statistically significant（ χ^2= 106. 3,P〈0. 001）; multidrug-resistant patients were found most in relapse cases,accounting for 51. 2%. 53 cases received multidrug- resistant therapy with a recovery rate of 57. 1%. Conclusion Relapse patients were the key objects of the screening of multi drug resistance,and the treatment success rate was guaranteed by the standard treatment management of multi drug resistant tuberculosis patients.
