按动牵伸法为主推拿治疗膝关节骨性关节炎临床研究

Clinical Research on the Treatment of Knee Osteoarthritis by Pressing and Stretching Manipulation

目的:探讨按动牵伸法治疗膝关节骨性关节炎的疗效。方法:选取自2012年至2014年就诊于北京按摩医院的膝关节骨性关节炎病人55例,随机分成治疗组(基本手法结合按动牵伸法组)和对照组(基本手法组),观察比较治疗前后两组ISOA评分、SF-36评分和B超下膝关节腔积液前后径。结果:与对照组相比,治疗组总有效率较高(P<0.05);ISOA评分降低更明显(P<0.05),SF-36评分增高优于对照组(P<0.05);治疗前后关节腔积液前后径降低(P<0.001),与对照组比较无显著差异(P>0.05)。结论:"按动牵伸法"治疗膝关节骨性关节炎效果显著,在改善症状、提高患者生存质量方面优于基本手法。

Objective： To discuss the effect of pressing and stretching manipulation on KOA compared with the effect of fundamental manipulation. Methods： 55 KOA patients who visited Beijing Massage Hospital from 2012 to 2014 were divided into a treatment group（ pressing and stretching manipulation combined with fundamental manipulation） and a control group（ fundamental manipulation only）. Compare ISOA value,SF- 36 value and anteroposterior diameter of effusion by B- mode ultrasonic exam. Results： The total effective rate in the treatment group was higher than that in the control group. The difference was statistically significant. ISOA in the treatment group was descended after treatment. Compared with the control group,the treatment group was more effective. SF- 36 value was increased in the treatment group after treatment,and the difference was statistically significant（ P〈0. 05）. The treatment group was more effective than the control group. The anteroposterior diameter was decreased in the treatment group after treatment. The difference was statistically significant（ P〈0. 001）. The difference of descended value between two groups had no statistically significance（ P〉0. 05）.Conclusion： Pressing and stretching manipulation is more effective than fundamental manipulation,especially for relieving symptoms and improving patients＇ living qualities.
