摘要
目的分析手持测压表间断测压法导致气管导管套囊内压力损失的原因,为临床工作中正确使用测压表提供参考。方法实验研究,在气管模型中,使用测压表间断测量气管导管套囊内压力,通过控制三通管分别测量断开及连接指示气囊导致的套囊内压力损失值和测压表导致的套囊内压力损失值。临床研究,选取重症医学科建立人工气道的患者作为研究对象,用手持测压表测量其气管导管套囊内压力。结果实验研究和临床研究中测压表间断测量所致的套囊内压力损失值分别为(10.18±0.52)cm H_2O和(10.27±0.93)cm H_2O,两者比较,差异无统计学意义(P>0.05);实验研究显示断开及连接指示气囊导致的套囊内压力损失值为(1.23±0.53)cm H_2O,手持测压表导致的套囊内压力损失值为(9.18±0.54)cm H_2O。结论间断测量过程中造成的套囊内压力损失值约等于测压表导致的压力损失值与断开及连接指示气囊导致的压力损失值之和。
Objective To analyze causes of pressure loss in HVLP cuff caused by intermittent measurement of handheld gauge and to provide references for the clinical work. Methods Experimental research: the endotraeheal tube (ETT) was inserted into a trachea model,and the pressure measurement was carried out. By switching the tee joint,we measured the pressure loss when connecting and disconnecting the indicator cuff and the pressure loss through the gauge,separately. Clinical research:ICU patients with artificial airways were involved in this study,and we measured the pressure loss of the endotracheal tube. Results The pressure loss caused by intermittent measurement of the 2 researches were(10.18±0.52) cmH2O and(10.27 ±0.93) cmH2O,and the difference had no statistical significance(P〉0.05);the pressure loss when connecting and disconnecting the indicator cuff was( 1.23± 0.53) cmH2O; the pressure loss though the gauge was(9.18±0.54) cmH2O. Conclusion The total pressure loss during intermittent measurement of the endotracheal intubation cuff is equal to the value of the loss caused by the pressure gauge and the pressure loss when connecting and disconnecting the indicator cuff.
