CT引导下背根神经节阻滞治疗带状疱疹后神经痛的疗效观察及对IL-6的影响 被引量:12

摘要 目的:观察CT引导下背根神经节阻滞治疗带状疱疹后神经痛(postherpetic neuralgia,PHN)的临床疗效及对血清IL-6的影响。方法:选择PHN患者40例,按照随机数字表法分为对照组和治疗组(n=20)。对照组采用CT引导下安慰剂注射及药物治疗;治疗组采用CT引导下背根神经节阻滞及药物治疗。观察治疗前1天、治疗后1天、1周、4周、8周、24周的疼痛视觉模拟评分(visual analogue scale,VAS)、加巴喷丁及曲马多的用药量及血清IL-6水平。结果:治疗后两组VAS评分、口服药物量及血清IL-6水平逐渐降低;治疗组治疗后VAS评分、口服药物量及IL-6水平较对照组显著下降(P<0.05);血清IL-6水平与VAS评分显著相关(P<0.05)。结论:CT引导下背根神经节阻滞能有效缓解PHN患者的疼痛,并能明显降低血清IL-6的水平。 Objective: To observe the clinical effect of CT-guided dorsal root ganglion block in the treatment of PHN and its effect on serum IL-6. Methods: Forty cases of patients with PHN were selected and randomly divided into control group and treatment group(n = 20 in each group). The control group was treated with CT-guided placebo injection and drug therapy, and the treatment group was treated with CT-guided dorsal root ganglion block and drug therapy. The VAS, the dosage of gabapentin and tramadol, and the serum IL-6 level of patients were observed before treatment, 1 day, 1 week, 4 weeks, 8 weeks, and 24 weeks after treatment. Results: The VAS, the dosage of tramadol and gabapentin, and the serum IL-6 were gradually decreased after treatment in both groups, but more decrease was found in the treatment group compared with the control group(P 0.05). The VAS was correlated with the serum IL-6 level. Conclusion: CT-guided dorsal root ganglion block can rapidly and significantly alleviate neuropathic pain of patients with PHN, and the treatment can also reduce the level of serum IL-6.
