分次灌注高黏度骨水泥行椎体后凸成形术治疗骨质疏松性椎体压缩骨折效果分析 被引量:4

摘要 目的 探讨分次灌注高黏度骨水泥行椎体后凸成形术治疗骨质疏松性椎体压缩骨折的疗效。方法 34例(34个椎体)骨质疏松性椎体压缩骨折患者,应用分次灌注高黏度骨水泥行椎体后凸成形术治疗术前、术后24h、术后1个月及末次随访采用视觉模拟评分(VAS)评估腰背部疼痛,Oswestry功能障碍指数(ODI)评估腰背部功能,观察椎体前缘高度、后凸Cobb角变化。结果 术后24h、术后1个月及末次随访,VAS评分及ODI指数均小于术前(均P〈0.05),术后椎体前缘高度明显恢复(均P〈0.05),伤椎高度恢复率术后24 h最高,与术后1个月及末次随访差异均有统计学意义(均P〈0.05)。术后Cobb角明显降低(均P〈0.05),后凸矫正率术后1个月最高,与术后24 h及末次随访差异均有统计学意义(均P〈0.05)。本组患者均获随,访随访时间6-34个月,3个椎体出现骨水泥渗,漏渗漏入椎间盘内2例,椎体前侧1例,均未出现神经症状。1例L1椎体压缩性骨折,术后3个月出现T(12)再骨折,再次行椎体后凸成形术治疗。结论分次灌注高黏度骨水泥行椎体后凸成形术治疗骨质疏松性椎体压缩骨折,可有效恢复压缩椎体的高度,迅速缓解疼痛,且有助于防止骨水泥渗漏,值得临床推广。
