CT在诊断与鉴别肺部真菌感染中的临床价值分析 被引量:4

摘要 目的研究CT应用于肺部真菌感染诊断与鉴别中的临床价值。方法以该院2015年4月—2016年3月的38例肺部真菌感染患者作为观察组,再以20例肺结核患者作为对照组A,20例肺癌患者作为对照组B。使用多层螺旋CT对所有患者进行扫描,以穿刺活检为金标准。分析观察组不同感染真菌的分布情况、病灶形态特征及诊断结果符合情况;对比3组的CT征象及病灶CT增强情况。结果观察组CT检查结果符合概率为52.63%;多数真菌感染病灶中均会出现相同特征,仅曲霉菌感染会出现新月征特有表现;3组CT征象均有多种表现并存现象,难以区分;在病灶CT增强情况方面,观察组CT增强在61-80 HU范围内数据为47.37%,较另2组更高,P〈0.05。结论 CT诊断应用于肺部真菌感染中,对于真菌类别的鉴定局限性较大;在诊断方面需注意与其他疾病区分,采用病灶增强CT操作可提高诊断概率。 Objective To study the clinical value of CT in the diagnosis and differentiation of pulmonary fungal infections. Methods 38 cases of pulmonary fungal infection in our hospital from April 2015 to March 2016 as the observation group, 20 cases of pulmonary tuberculosis patients as control group A, 20 cases of lung cancer patients as control group B. All patients were scanned with multislice spiral CT in order to puncture biopsy as the gold standard. Analysis of the distribution of different infection fungi in the observa-tion group, the morphological characteristics and the diagnostic results were consistent with the three groups of CT signs and lesions CT enhancement. Results The observation group CT examination results are consistent with the probability of 52.63%; the same fea-tures are most lesions of fungal infection, only Aspergillus infection will appear crescent sign unique performance of three groups; CT signs were various manifestations of the phenomenon exist, it is difficult to distinguish; enhancement in lesions of CT, CT in the ob-servation group was enhanced in 61-80 HU range the data was 47.37%, higher than the other two groups, P〈0.05. Conclusion CT diagnosis of pulmonary fungal infection, the identification of the fungal classification of the larger; in the diagnosis of the need to pay attention to the diagnosis and other diseases, the use of enhanced CT operation can improve the probability of diagnosis.
