顺行V-Y形岛状瓣修复指端缺损30例疗效分析

Anterograde V-Y-shaped Island Flap in Fingertip Defect Clinical Analysis of 30 Cases

目的探究顺行V-Y形岛状瓣修复指端缺损的应用效果。方法选取该院2012年6月—2015年12月收治的60例指端缺损患者,按照治疗方式的不同分为对照组和观察组各30例,对照组采用逆行岛状皮瓣修复术治疗,观察组采用顺行V-Y形岛状瓣修复术治疗。比较两组伤口愈合时间、皮瓣成活及皮瓣并发症发生情况,对观察组30例患者术后随访情况进行记录。结果比较两组皮瓣成活率和伤口愈合时间,对照组为93.3%和(16.6±1.3)d,观察组为100.0%和(16.2±0.5)d,对比差异无统计学意义(P>0.05);比较两组皮瓣并发症发生情况,对照组发生率为23.3%,观察组发生率为3.3%,对比差异有统计学意义(P<0.05)。观察组术后随访,皮瓣均无色素沉着、色泽红润,指腹饱满且质地较为柔软,指端存在指纹,外形较为美观,患指感觉功能均达到S4级。结论逆行岛状皮瓣修复术与顺形V-Y形岛状瓣修复术治疗指端缺损均有疗效,但顺形V-Y形岛状瓣修复术皮瓣并发症发生率低,治疗效果安全可靠,因此在临床治疗中具有重要意义。

Objective To explore along the line V-Y-shaped island flap application effect of fingertip defect. Methods In our hospital in June 2012 to December 2015 were treated 60 cases of fingertip defect patients, according to different treatment modalities were divid-ed into control group and observation group, 30 patients in the control group were treated with reverse island flap for treatment, obser-vation group anterograde VY-shaped island flap repair treatment. Comparison of the two groups wound healing, skin flap and flap complications were 30 cases in the observation group patients were followed up cases recorded. Results Compared flap survival and wound healing time, and 93.3% in the control group（16.6 ± 1.3） d, the observation group was 100.0% and（16.2 ± 0.5） d, contrast no significant difference（P〉0.05）; flap complications were compared between two groups, the control group, the rate was 23.3%,in the observation group the rate was 3.3%, compared to having a statistically significant difference（ P〈0.05）. Observation of postoperative follow-up, the flap no pigmentation, ruddy color, softer texture and full of pulp, finger fingerprint exists, more beautiful shape, suffer-ing from sensory function refers to meet the S4 level. Conclusion Island flap repair cis form VY-shaped island flap for treatment of fingertip defect are effective, but conformal VY-shaped island flap of flap complication rate, treatment is safe and reliable, so It has important significance in clinical treatment.
