胃癌单纯手术与化疗、手术综合治疗方法疗效比较

Efficacy of Operation and Chemotherapy of Gastric Cancer

目的：探讨化疗辅助手术治疗进展期胃癌的临床效果.方法：2011年10月～2012年10月收集进展期胃癌患者78例随机分组,化疗辅助组39例手术前辅助化疗治疗,单纯手术组39例单纯采用手术治疗.结果：辅助化疗组患者肿瘤切除率为87.2％,根治性切除率61.5％;单纯手术组肿瘤切除率66.7％,根治性切除率38.5％,辅助化疗组数据明显高于单纯手术组,两组间比较具有统计学差异（P＜0.05）.辅助化疗组患者治疗后生存质量评分为（76.2＆#177;13.8）分,单纯手术组生存质量评分（62.3＆#177;12.2）分,辅助化疗组数据明显高于单纯手术组,两组间比较具有统计学差异（t=4.712,P＜0.01）.辅助化疗组患者化疗期间主要表现为胃肠道、神经毒性和骨髓抑制性不良反应,发生比例分别为61.5％、66.7％和46.2％,但患者不良反应均以轻度为主,经对症处理后改善,未影响治疗.结论：胃癌治疗应用术前辅助化疗和手术综合治疗具有较好的疗效,可提高患者生存质量,化疗采用奥沙利铂和5-氟尿嘧啶副反应轻,安全性好.

OBJECTIVE ： To investigate the clinical effect of chemotherapy in the treatment of advanced gastric cancer operation. METHOD： 78 patients with gastric cancer from October 2011 to October 2012 were randomly divided into chemotherapy group （n = 39） and operation group （n = 39）. RESULTS： In chemotherapy group tumor resection rate was 87.2% and radical resection rate was 61.5%. In operation group tumor resection rate was 66. 7% and radical resection rate was 38.5%. Data in the chemotherapy group was significantly higher than that in simple operation group. There was statistical difference between the two groups （ P 〈 O. 05 ）. In adjuvant chemotherapy group patients＇ quality of life score was （76. 2 ~ 13.8 ）% and in simple operation group （62. 3 ＋ 12. 2 ）%. There was statistical difference （ t = 4. 712,P 〈0. 01 ） between two groups. During the chemotherapy the patients in chemotherapy group mainly had gastrointestinal, neurotoxicity and myelosuppression adverse reactions, respectively 61.5% , 66.7% and 46.2%. But the adverse reactions of these patients were mild. CONCLUSIONS ： Chemotherapy and operation of gastric cancer have better cura- tive effect and can improve the quality of life of those patients. Oxaliplatin and 5 - fluorouracil only have mild side effects.
