摘要 目的探讨64排螺旋CT对活动性肺结核的诊断价值。方法选择经临床和实验室检查确诊的186例肺结核患者的临床资料,包括男性126例,女性60例;年龄16~54岁,平均年龄35岁。其中活动性肺结核86例,非活动性肺结核100例。回顾性分析其多层螺旋CT征象。结果活动性肺结核常见的CT表现为:树芽征72例(83.7%),小叶中心结节影70例(81.4%),磨玻璃样密度影65例(75.6%),斑片影62例(72.1%),空洞58例(67.4%)。非活动性肺结核常见的CT表现为:不规则线状影89例(89.0%),钙化结节78例(78.0%),支气管壁增厚47例(47.0%)。结论活动性肺结核有其较明显的CT征象,多层螺旋CT对肺结核活动性的判断有重要的临床应用价值。 Objective To investigate the value of 64-row helical CT in diagnosing active pulmonary tubereuloma. Methods A total of 186 pulmonary tuberculosis patients confirmed by clinical and laboratory examination were enrolled, which included 126 males and 60 females, aged 16 - 54 years old with mean age of 35 years old. Among these cases, 86 with active pul- moary tuberculoma and 100 with inactive pulmonary tuberculoma. The CT signs of all the cases were analyzed retrospectively. Results Common CT signs of active tuberculosis were that tree in bud appearance of 72(83.7 %), centrilobular nodules with segmental distribution of 70(81.4 %), ground- grass signs of 65(75.6 %), patching-shadowing of 62(72.1%) and cavity of 58(67.4 %). Common CT signs of inactive tuberculosis were that irregular linear shadows of 89(89.0 %), calcified nodules of 78(78.0 %) and thicken bronchial wall of 47(47.0 %). Conclusion It is demonstrated that CT signs of active pulmonary tuberculosis shows ob- vious signs, which have important clinical value for judgment activity of pulmonary tuberculosis.
