动机性访谈式干预对有机磷中毒性肌病患者心理状态的影响

Effects of motivational interviewing on the psychology of patients with organophosphorus toxic myopathy

目的探讨动机性访谈（motivationa linterviewing，MI）对有机磷中毒性肌病患者心理状态的影响。方法确诊为有机磷中毒性肌病的患者100例，随机均分为对照组例（给予常规治疗和护理）和访谈组（在常规治疗和护理的基础上，给予每次20min的动机访谈和20min的心理疏导，每周访谈2次，为期4周）。用症状自评量表（SCL-90）评价心理状态和治疗依从性。结果MI组躯体化、人际关系、强迫、抑郁、焦虑、恐怖、偏执性因子得分均显著低于常模（均P〈0．05）；而对照组躯体化、人际关系、强迫、抑郁、焦虑、恐怖因子得分均显著高于常模（均P〈O．05）。MI组应对方式各因子得分均高于对照组（均P〈0．05）。MI组康复治疗依从性好的患者百分率显著高于对照组（P〈0．05）。结论MI能有效改善有机磷中毒性肌病患者的心理状态，改善其生活方式，提高患者治疗依从性。

Objective To investigate the effects of motivational interviewing （MI） on the psychology of patients with organophosphorus toxic myopathy. Methods 100 patients with organophosphorus toxic myopathy were randomly divided into 2 equal groups： control group given conventional treatment , and motivational interviewing （MI） group given MI and psychological counseling 20 rain each twice a week for 4 weeks in addition to routine treatment and nursing. Symptom checklist 90 （SCL-90） was used to evaluate the psychology of the patients. Results The scores of somatization, interpersonal relationship, compulsion, depression, anxiety, terror, and paranoid factor of the MI group were all significantly lower than those of the Chinese norm （all P 〈0.05）; and the scores of somatization, interpersonal relationship, compulsion, depression, anxiety, and terror of the control group were all signioficantly higher than those of the Chinese norm （all P 〈0.05）. The scores of the factors of coping styles in the MI group were all significantly higher than those of the copntrol group （all P 〈 0.05）. The pervcentage of the patients with good compliance to rehabilitation therapy in the MI group was significantly higher than that of the control group （P 〈 0.05）. Conclusion MI effectively improves the mental status of the patirents with organophosphorus toxic myopathy and the patients＇ compliance.
