WU Weiping's Experiences of Disease,Syndrome,Symptom Differentiation in Treating Chronic Obstructive Pulmonary Disease

介绍武维屏运用病证症结合辨治模式治疗慢性阻塞性肺疾病的经验。武维屏提出因病审机论治,审病机虚痰瘀为关键,论治法补虚化痰行瘀理肺四法多用。论辨证,分急缓两期七证候,急重期六种证候较常见,稳定期气虚血瘀痰阻贯穿始终;析证候,从三级十症别轻重,注重咳、痰、喘、昏、脱之变化。从咳论治,当分寒、热、燥、湿、暑、风、气、血、阴、阳;从痰论治,需分寒、热、燥、湿、郁、瘀;从喘论治,分急缓,急辨五候,缓别悸喘、痰喘、肿喘、绀喘;从昏论治,痰热阻窍、痰浊蒙窍、热扰心营三种证候最常见;从脱论治,分阴阳之别。

WU Weiping＇s experiences of disease,syndrome,symptom differentiation in treating chronic obstructive pulmonary disease were introduced in this paper. Professor WU proposed treatment according to etiology and pathogenesis among which deficiency,phlegm and blood stasis were the key points. Tonifying the deficiency,resolving phlegm and promoting blood circulation,regulating lung Qi are often used in treatment. On syndrome differentiation,lt can be divided into two stages and seven syndromes. In the acute exacerbation stage,six syndromes are common. In the stable stage,Qi deficiency,blood stasis,phlegm obstruction always exist from beginning to the end. On syndrome,three grades and ten syndromes can be distinguished to determine the severity, Attention should be paid on the changes of cough,phlegm,pant,syncope,prostration. On the treatment of cough,we should differentiate cold, heat, dryness, dampness, summer-heat,wind, Qi,blood, Yin,Yang. On the treatment of phlegm,we should distinguish cold,heat, dryness, dampness,melancholia,blood stasis. On the treatment of pant,we should differentiate the acute and chronic diseases. The acute stage can be divided into five syndromes. The chronic stage should be distinguished palpitate pant,phlegm pant,edema pant,cyanosis. On the treatment of syncope,phlegm heat obstructing the orifice,phlegm and turbidity ob- structing orifice,heat disturbing heart spirit are common. On the treatment of prostration,we should distinguish Yin and Yang.
