穴位注射合并火针治疗复发性口腔溃疡326例临床观察

Clinical Observation of Point-injection and Fire Needle in Treating 326 Patients with Recurrent Oral Ulcer

目的 :观察穴位注射合并火针疗法辨证治疗复发性口腔溃疡的临床疗效并探讨其疗效机制。方法 :将326例复发性口腔溃疡患者随机分为治疗组和对照组,治疗组根据不同证型选用中药注射液穴位注射,每周3次,共治疗2周;火针点刺溃疡面及周围黏膜,每日1次,共治疗3次。对照组口服维生素C 100 mg、维生素B220 mg,每日3次;肌肉注射转移因子,每次2 m L(3 mg),每周2次,共治疗2周。观察两组患者治疗前后症状体征积分、疼痛视觉模拟量表(VAS)评分的变化,以及治疗后溃疡愈合时间、溃疡复发情况,并比较两组临床疗效。结果:治疗组总有效率为94.48%,优于对照组的76.07%(P<0.01)。治疗后治疗组症状体征积分及VAS疼痛评分较对照组明显减少,差异有统计学意义(P<0.01)。治疗组溃疡愈合时间与对照组比较,差异有统计学意义(P<0.01)。治疗组患者复发率与对照组比较,差异有统计学意义(P<0.01)。结论:穴位注射配合火针疗法辨证治疗复发性口腔溃疡疗效优于口服维生素及肌肉注射转移因子,其止痛效果好、见效快,可改善相关症状,缩短溃疡愈合时间,降低复发率。

Objective：To observe the clinical efficacy of point-injection combined fire needle in treating recurrent oral ulcer and to explore the mechanism. Methods ：Total 326 patients with recurrent oral ulcer were randomized to the treatment group and control group,163 patients in each group. Then the treatment group received point injection of Chinese medicine according to different syndrome types,3 times per week for 2 weeks and fire needle around the ulcer surface and mucous membranes, 1 times a day for 3 times. The control group was given oral vitamin C 100 mg, vitamin B2 20mg,3 times a day,intramuscular injection of transfer factor 2 mL（3 mg）,2 times a week for 2 weeks. The changes of symptom and sign scores and VAS scores of the two groups before rences were observed to compare the clinical curative and after treatment,ulcer healing time and the recur- effects of the two groups. Results ：The total effective rate of the treatment group was 9d.48% better than 76.07% in the control group（P〈0.01）. The symptom and sign scores and VAS scores of treatment group were less than the scores in the control group after treatment,the difference was statistically significance（P〈0.01）. Ulcer healing time in the two groups was compared with statistical significance（P〈0.01）. The recurrence rate in the two groups was compared with statistical significance（P〈0.01）. Conclusion：Curative effects of point-injection combined fire needle in treating recurrent oral ulcer were better than oral vitamin and muscle injection of transfer factor,the analgesic effect was good with quick effects,could improve the pain and related symptoms,shorten the ulcer healing time, reduce the recurrence rate significantly.
