六西格玛管理系统对ICU住院患者发生肺部感染的防护作用研究

Research on the protective effect of Six Sigma Management system on pulmonary infection in hospitalized patients in department of ICU

目的对ICU住院患者实施六西格玛管理措施,探讨六西格玛管理对预防患者出现肺部感染的影响。方法选择本院ICU2015年1—12月间120例住院患者,其中60例实施传统管理模式,设为传统组,另外60例实施六西格玛管理方法,设为改进组;同期选取本院ICU医护人员34人,对比分析住院患者对临床护理质量评价及科室工作人员对管理模式的满意度,并观察肺部感染发生情况。结果在护理质量方面,改进组患者对临床提供的操作实施、护理人员无菌意识、护患沟通及护理服务态度等指标评分,均明显高于传统组,差异有统计学意义（P〈0.05）;对ICU住院患者实施传统管理模式,科室工作人员对护理模式管理满意度明显低于实施六西格玛管理方法,差异具有统计学意义（P〈0.05）;对ICU 60例住院患者实施传统管理模式有14例患者出现肺部感染,感染率为23.3%（14/60）,而60例ICU住院患者实施六西格玛管理模式有3例患者出现肺部感染,感染率为5.0%（3/60）,两组感染率之间差异有统计学意义（χ^2=17.316,P〈0.01）。结论针对ICU收治的住院患者而言,总结导致其出现肺部感染相关因素并制定相关改进管理措施,可提高患者对临床服务质量评分,且科室护理人员主观对管理模式感到满意,科室出现肺部感染几率较小。

Objective To implementing six-sigma management measures in ICU inpatients,and discuss the influence of six- sigma management on preventing the patients from suffering lung infection. Methods 120 cases of patients hospitalized in ICU of our hospital during January and December 2015 were select. They were divided into traditional and improvement group. The 60 cases of the traditional group adopted traditional management mode,the other 60 cases in improvement group implementing six-sigma management model. During this period,34 staffs in ICU of our hospital were selected in this study. Compared and analyzed the evaluation of hospitalized patients on clinical nursing quality,the satisfaction of the staff on this management model,and observed the situation of lung infection. Results In terms of quality of care, the scores of operation implementation,sterile awareness of nurses,communication between nurse and patients,nurses and nursing service attitude in improvement group were significantly higher than that of traditional group,the differences were statistically significant（ P〈0.05）. The staffs in ICU were more satisfied with six-sigma management method,the difference was statistically significant（ P〈0. 05）. 14 cases of patients in traditional management suffering from pulmonary infection,the infection rate was 23. 3%（ 14 / 60）,however,there were only three cases of lung infection in six-sigma management group,and the infection rate was 5.0%（ 3 / 60）. Infection rates between the two groups had statistically significant difference（ χ^2= 17.316,P〈0.01）. Conclusions For hospitalized patients in ICU,summarize related factors which lead to pulmonary infection and formulate the relevant improvement measures,can improve the quality of clinical service,and nursing staffs were subjective satisfied with the management mode,the occurrence rate of lung infection low.
