基于微糖平台的健康管理模式对2型糖尿病患者的影响

Influence on patients with type 2 diabetes by using individual Health Management Model based on Micro sugar Platform

目的评价基于微糖平台的健康管理模式对2型糖尿病患者知识水平、自我管理能力及糖化血红蛋白的影响。方法将有效例数138例2型糖尿病患者按照随机数字表法分为试验组70例和对照组68例。试验组采用微糖平台的健康管理模式,对照组采用传统的电话随访模式。于入组前、入组后3个月、入组后6个月采用密西根糖尿病知识测试问卷、糖尿病患者自我管理行为量表评价糖尿病患者知识水平和自我管理水平,并测定空腹血糖(FBG)、餐后2 h血糖(2h FBG)及糖化血红蛋白(HbA1c)水平。结果入组后糖尿病患者知识得分、SDSCA总分、饮食治疗得分、运动治疗得分、药物治疗得分、血糖监测得分、足部护理得分均较入组前升高(P<0.05);入组后患者FBG、2 hPBG、Hb A1c水平均较入组前降低(P<0.05)。基于微糖平台的健康管理模式糖尿病患者的知识水平和自我管理水平提高显著高于传统的电话随访模式。结论基于微糖平台的健康管理模式有助于糖尿病患者提高知识水平和自我管理水平,降低糖化血红蛋白水平。

Objective To evaluate the effect of Micro sugar Platform based individual Health Management Model on knowledge level,self-management ability and Hb Al C of type 2 diabetes patients.Methods The 138 cases of type2 diabetes patients were randomly assigned to the experimental group（ n = 70） and the control group（ n = 68）. The experimental group adopted micro sugar platform based health management mode,while the control group used traditional telephone follow- up mode. Michigan diabetes knowledge test questionnaires and diabetes self- management behavior scale were used to evaluate the knowledge and self-management of patients with diabetes at the time points before enrollment,3 months after enrollment and 6 months after enrollment. The concentration of FBG,2hFBG and HbA1c of patients were determined at the same time.Results After the intervention,the knowledge score,SDSCA score,diet therapy score,exercise therapy score,medication score,glucose monitoring score and foot care score of diabetes patients raised,significantly higher than those before the intervention（ P〈0. 05）. However,the concentration of FBG,2hPBG and HbA1c decreased after the intervention. The knowledge level and self- management level of patients in experimental group significantly higher than those in control group. Conclusions The Health Management Model based on Micro sugar Platform helps to improve the Diabetes knowledge level and individual health self-management,and can reduce glycated hemoglobin levels.
