CysC在中老年CKD患者肾功能评价中的应用

Application of CysC in the Evaluation of Renal Function in Elderly Patients With CKD

目的探讨在中老年慢性肾脏病(CKD)患者中进行肾功能评价时胱抑素C(CysC)的应用价值。方法回顾性分析144例中老年CKD患者,以肾小球滤过率60 ml/(min·1.73 m2)(99 mTc-DTPA肾动态显像法测定r GFR)将患者区分为肾功能正常及异常;同时使用CysC、Scr、BUN及基于Scr为基础的CKD-EPI方程估算GFR(EPIScr)评估。比较CysC、Scr、BUN及EPIScr在中老年及不同性别人群中的灵敏度、准确度及特异度;并分析其与r GFR的一致性。结果在中老年及不同性别人群中CysC的灵敏度和准确度均占优势,但特异度方面4者间无明显差异;CysC与r GFR的相关度在不同人群中均最高。结论 CysC是中老年人群肾功能评估的重要指标。

Objective To study the value of cystatin C（CysC） for the evaluation of renal function in middle aged and old patients with chronic kidney disease（CKD）. Methods A retrospective analysis was carried out on 144 elderly patients with CKD. Glomerular filtration rate（GFR） was calculated from 99mTc-DTPA renal dynamic imaging method（r GFR）, and we judged whether the anomaly by 60 ml/（min·1.73 m2）. At the same time, CysC, Scr, BUN and GFR caculated by CKD-EPI equation which based on Scr（EPIScr） were used to evaluate renal function. To compare the sensitivity, accuracy and specificity of CysC, Scr, BUN and EPIScr in the elderly and different gender groups, and to analyze their consistency with r GFR.Results The sensitivity and accuracy of CysC in the middle and old age and gender were dominant, but there was no significant difference between the four groups. The correlation between CysC and r GFR was the highest in different groups. Conclusion CysC is important for the evaluation of renal function in middle aged and old people.
