摘要
目的观察中暑患者血液中心肌酶谱的变化,探讨中暑患者心肌酶谱变化的临床意义。方法选择61例住院中暑患者,按临床表现划分轻症中暑组(n=28)和重症中暑组(n=33),于入院24 h内检测外周血中心肌酶谱(门冬氨酸氨基转移酶AST,乳酸脱氢酶LDH,磷酸肌酸激酶CK,肌酸激酶同工酶CKMB),同时随机抽取30例健康体检者作对照,比较三组观察对象心肌酶谱的变化。结果两组中暑患者AST、LDH、CK、CKMB均高于健康对照组,且重症中暑组AST、LDH、CK、CKMB均高于轻症中暑组。中暑患者AST、AST、LDH、CK、CKMB与APACEHⅡ评分呈负相关。结论早期检测中暑患者心肌酶谱可能有助于评估其疾病严重程度。
Objective To observe the changes of myocardial enzymes in patients with heat stroke, and its clinical significance.Methods61 cases of inpatients with heat stroke were divided into the mild heat stroke group (n=28) and severe heat stroke group (n=33) according to the clinical manifestations. Within 24 hours after admission, the peripheral blood myocardial enzymes [aspartate aminotransferase (AST), lactate dehydrogenase (LDH), creatine kinase (CK), creatine kinase (CKMB)] were detected. Another 30 healthy subjects were selected as control, and changes of myocardial enzymes were compared between groups.Results AST, LDH, CK and CKMB in two heat stroke groups were higher than those in healthy control group, and those in severe heat stroke group were higher than the mild heat stroke group. AST, LDH, AST, CK and CKMB were negatively correlated with APACEH II score in patients with heat stroke.Conclusion Early detection of myocardial enzymes is helpful to evaluate the severity of heat stroke in patients.
