138例过敏性紫癜患儿免疫与临床观察

Immune and Clinical Observation of 138 children with Allergic Purpura

目的观察HSP患儿与对照组及紫癜肾炎组与无肾炎组的免疫状态。方法年龄均为2~13岁;各组进行统计学t检验。结果急性期与对照组比较lg A、lg M、CIC、C3,有显著差异（P〈0.01）。CD3＋、CD4＋、CD8＋均下降（P〈0.05）,而CD4＋/CD8＋比值有所下降,但无统计学意义。紫癜肾炎与无肾炎组之间的免疫球蛋白及T细胞亚群均无明显差异。结论HSP是一种以多克隆B细胞活化、增殖为特征的免疫调节异常性疾病;T细胞亚群失调和功能降低是HSP的重要发病因素;紫癜肾炎与无肾炎组之间不存在体液免疫与细胞免疫功能异常。

Objective The immune status of HSP patients with the control group,the purpura nephritis group and no nephritis group were observed.Methods The average age were between 2 and 13 years old;and were statistically checked.Results The acute stage was compared with the control group in lg A,lg M,CIC,C3,and there were significant differences（P〈0.01）.CD3 ＋,CD4 ＋,CD8 ＋ were decreased（P〈0.05）,and CD4 ＋ / CD8 ＋ ratio were decreased,but there was no statistical significance.Purpura nephritis with nephritis group between immune globulin and T cell subgroup had no obvious difference.Conclusion HSP is a characterized by polyclonal B cell activation and proliferation of immune dysregulation disease;Disorder of T cell subgroup and function decrease are important pathogenesis of HSP factors;Purpura nephritis with nephritis group does not exist between humoral immunity and cellular immune function abnormalities.
