颅脑超声诊断新生儿获得性脑损伤的临床价值 被引量:19

摘要 目的评价颅脑超声在新生儿获得性脑损伤中的诊断价值。方法对12 716例临床疑诊为脑损伤的新生儿行颅脑超声检查,以CT、MRI等诊断结果为标准,对超声提示颅脑损伤的病例进行分析。结果经反复颅脑超声、CT、MRI检查并结合临床表现及治疗随访超声诊断结果与最终结果相符12 633例。颅脑超声检出获得性脑损伤新生儿4885例,其中缺氧缺血性脑病2642例,脑室周围-室内出血1718例,脑室扩张及脑室积水416例,蛛网膜下腔出血48例,颅内感染34例,脑实质出血27例。超声诊断假阳性83例,假阴性20例。结论颅脑超声对新生儿获得性脑损伤具有重要诊断价值。 Objective To evaluate the value of ultrasound in diagnosis of neonatal acquired brain injury. Methods Cerebral ultrasound was applied on 12 716 children with clinical suspected brain damage. Taking CT,MRI results as the gold standard, the cases of craniocerebral injury diagnosed by ultrasound were analyzed. Results Twelve thousand six hundred and thirty-three cases were confirmed by CT,MRI combined with clinical appearance and treatment, 4885 cases of acquired brain injury were diagnosed by cerebral ultrasound, including 2642 cases of hypoxic ischemia encephalopathy,1718 cases of periventricular-intraventricular hemorrhage,416 cases of water in the expansion and ventricle,48 cases of subarachnoid hemorrhage,34 cases of intracranial infection,27 cases of parenchymal hemorrhage. There were 83 false positive cases and 20 false negative cases.Conclusion Eonatal cerebral ultrasound has important clinical value in diagnosis of neonatal acquired brain injury.
