摘要
目的探讨分析血浆脂蛋白磷脂酶A_2(LP-PLA_2)水平在急性动脉硬化性脑梗死患者中的变化及临床价值。方法选取115例急性动脉硬化性脑梗死患者,同时选取60例对照组患者,比较两组患者LP-PLA_2水平以及不同梗死面积、不同神经功能缺损程度患者LP-PLA_2水平的变化,并分析LP-PLA_2水平与患者年龄、性别、体质指数(BMI)、吸烟、收缩压(SBP)、舒张压(DBP)、血清总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)及C反应蛋白(CRP)之间的相关性。结果病例组患者的LP-PLA_2水平较对照组人群显著提高,大面积梗死的患者LP-PLA_2水平较小面积梗死的患者显著提高,不同程度神经功能缺损的患者LP-PLA_2水平差异具有统计学意义(P<0.05),进一步分析,随着患者神经功能缺损程度的加重,LP-PLA_2水平逐渐升高,患者性别、吸烟、SBP、DBP、CRP与患者的LP-PLA_2水平无显著性相关(P>0.05),患者年龄、BMI、TC、TG、LDL-C与LP-PLA_2水平呈正相关(P<0.05),患者的HDL-C与LP-PLA_2水平具有显著性负相关(P<0.05)。结论 LP-PLA_2与急性动脉硬化性脑梗死患者的梗死功能面积、神经功能缺损程度及生化指标密切相关,通过测定急性动脉硬化性脑梗死患者的LP-PLA_2能够预测疾病的发展,具有一定的临床价值。
Objective To analyze the changes and clinical value of plasma lipotrotein phospholipase A2 level ( LP-PLA2 ) in pa-tients with acute arteriosclerosis cerebral infarction ( ACI ) . Methods 115 cases with acute ACI were selected as the observation group, and 60 other cases were selected as the control group. The LP-PLA2 levels in two groups were compared and the changes of LP-PLA2 levels in patients with different infarct sizes and different degrees of neurological deficits were compared. In addition, the corre-lation of LP-PLA2 levels with patients’ age, sex, body mass index ( BMI) , smoking, systolic blood pressure ( SBP) , diastolic blood pressure ( DBP) , total cholesterol ( TC) , triglyceride ( TG) , high density lipoprotein cholesterol ( HDL-C) , low density lipoprotein cholesterol ( LDL-C) and C-reactive protein ( CRP) was analyzed. Results Compared with the control group, the LP-PLA2 level of the observation group increased significantly. LP-PLA2 level in patients with large infarcts increased significantly compared with that in patients with smaller infarct size. The differences in LP-PLA2 level among patients with varying degrees of neurological impairment were of statistical significance (P〈0. 05). To be further, LP-PLA2 level gradually increased as patientsˊneurological impairment ag-gravated. Patients’ LP-PLA2 level had no significant correlation with patients’ sex, smoking, SBP, DBP and CRP (P〉0. 05) and it had positive correlation with patients’ age, BMI, TC, TG and LDL-C (P〈0. 05). Patients’ HDL-C was in negative correlation with LP-PLA2 level (P〈0. 05). Conclusion LP-PLA2 is closely related to the infarct size, the degree of neurological deficit and biochemi-cal indexes of patients with acute arteriosclerosis cerebral infarction. By measuring LP-PLA2 level of patients with acute arteriosclerosis cerebral infarction, the development of the disease can be predicted. Therefore, it has certain clinical values.
