促性腺激素释放激素激动剂治疗子宫内膜异位症合并不孕腹腔镜术后40例

GnRH-α for Treating Endometriosis Associated Infertility After Laparoscopic Surgery in 40 Cases

目的探讨子宫内膜异位症合并不孕患者腹腔镜术后促性腺激素释放激素激动剂(Gn RH-α)治疗的妊娠结局及临床疗效。方法将医院2012年10月至2015年6月收治的子宫内膜异位症合并不孕患者80例随机均分为观察组和对照组,各40例,对照组单纯采用腹腔镜术治疗,观察组在此基础上加用Gn RH-α治疗,比较两组患者术后1年妊娠、复发情况及妊娠结局(自然流产、异位妊娠、早产、足月产、自然分娩、剖宫产)。结果观察组患者术后1年妊娠率为60.00%,显著高于对照组的32.50%;术后1年复发率为2.50%,显著低于对照组的17.50%(P<0.05)。观察组患者术后自然分娩率和剖宫产率与对照组比较,差异有统计学意义(P<0.05)。两组足月产率、早产率、异位妊娠率及自然流产率比较,差异均无统计学意义(P>0.05)。结论采用Gn RH-α治疗子宫内膜异位症合并不孕患者腹腔镜术后有助于提高妊娠率,减少复发,对妊娠结局具有一定影响,临床可推广应用。

Objective To investigate the pregnancy outcome and efficacy in patients with endometriosis associated infertility by the treatment of GnRH-αafter laparoscopic surgery.M ethods 80 patients with endometriosis associated with infertility in the hospital from October 2013 to June 2016 were randomly divided into the observation group（40 cases） and the control group（40 cases）.The control group was treated with laparoscopic operation,and the observation group was treated with gonadotropin releasing hormone agonist（GnRH-a） on the basis of the above treatment.The pregnancy and recurrence after 1 year,the pregnancy outcome（spontaneous abortion,ectopic pregnancy,premature delivery,term birth,natural childbirth,cesarean section） of two groups were compared.Results After one year,the rate of pregnancy rate in observation group（60.00%） was significantly higher than the control group（32.50%）;postoperative recurrence rate of 1 year（2.50%） was significantly lower than the control group（17.50%）（P〈0.05）.There was significant difference in the rate of spontaneous delivery and cesarean section rate between the observation group and the control group（P〈0.05）;there was no significant difference between the two groups in term birth rate,preterm labor rate,ectopic pregnancy rate and spontaneous abortion rate（P〉0.05）.Conclusion The treatment of GnRH-αafter laparoscopic surgery in patients with endometriosis associated infertility is helpful to improve the pregnancy rate and can reduce the recurrence.It has a certain impact on the outcome of pregnancy,which is worthy of clinical promotion.
