摘要
目的研究慢性乙醇中毒性周围神经病(CAPN)的神经电生理异常及脑执行功能。方法以CAPN患者为研究组,同时期健康志愿者为对照组,测定神经传导速度、动作电位潜伏期及波幅、尺神经F波、胫神经H反射,测定ODDBELL、STERBERG、双重视听任务的反应时间、正确率及追踪误差。结果 171.95%的CAPN患者存在神经电生理学异常。2所有CAPN患者的神经传导速度均显著偏低(t=10.186~30.143,P=0.000 0),动作电位潜伏期延长(t=7.299~31.806,P=0.000 0),动作电位波幅偏低(t=6.307~32.792,P=0.000 0)。3CAPN患者的任务反应时间显著延长(t=16.588~16.892,P=0.000 0),正确率显著偏低(t=8.499~23.782,P=0.000 0)。结论 CAPN患者在中枢神经系统临床表现出现前,已经存在脑部高级智能活动障碍,神经心理学检查有助于其早期诊断。
Objective To study the neuroprotenial abnormality and brain executive function of Chronic Alcoholism Peripheral Neuropathy(CAPN).Mothods CAPN sufferers were involved in the study group, and the healthy volunteers were taken as the control group.The nerve conduction velocity of peripheral nervous system,the delitescence and am- plitude of action potential, the F wave of ulnar nerve, and the H reflex of tibial nerve were detected in both groups, moreover the response time and correct rate in ODDBELL task, STERBERG task, and dual seeing and hearing task and tracking error were detected.Results (1)Neuroprotenial abnormality occurred in 71.95% of the CAPN sufferers.(2)Nerve conduction velocity of all CAPN sufferers was clearly low(t= 10.186~30.143, P=0.000 0),the delitescence of action potential prolonged (t =7.299 ~31.806, P =0.000 0), and the amplitude of action potential flatted (t = 6.307 ~32.792, P = 0.000 0). (3) The task response time of the CAPN sufferers clearly was prolonged (t = 16.588 ~16.892, P = 0.000 0), and the task correct rate was clearly lowered (t = 8.499 ~23.782, P = 0.000 0). Conclusion There are advanced intelligent activity obstacles in brains before the clinical manifestation of central nervous system in sufferers with chronic alcoholism peripheral neuropathy, and the rigorous neuropsychology examination can help with the early diagnosis.
