原发性脑出血住院患者直接经济负担及影响因素分析

Analysis of Direct Economic Burdens and Influence Factors of Inpatients with Primary Cerebral Hemorrhage

目的探讨原发性脑出血(PICH)的直接经济负担及相关影响因素,为卫生行政部门制定PICH的卫生服务政策提供科学的依据。方法采用回顾性队列研究的方式收集患者的相关信息资料,主要指标包括,人口学信息、疾病特征、住院费用、诊疗质量等。结果 PICH患者人均直接医疗总费用为12 483.20元,在患者的直接经济负担中,药费是第一位的负担,占48.87%,其次为手术费和治疗护理费,分别占24.25%和16.71%。直接经济负担的影响因素分析显示,影响PICH住院患者医疗总费用的主要因素有患者年龄、入院时GCS评分、住院天数、治疗方式。结论直接医疗费用中,药费比例过高;患者年龄、疾病严重程度、平均住院日、治疗方式的选择是影响直接经济负担的重要因素。

Objective To study the direct economic burdens and influence factors of inpatients with primary cerebral hemorrhage and provide scientific basis for making the health service policies by the health administrative department. Methods The related information data were collected by the retrospective research method, the main indicators mainly included the demography information, disease features, hospitalization cost and diagnosis and treatment quality. Results The per capita direct total medical cost of PICH patients was 12 483.20 yuan, in the direct economic burden, the drug fee was in the first place, accounting for 48.87%, the operation fee and treatment nursing cost were the second, respectively accounting for24.25% and 16.71%, and the analysis of influence factors of direct economic burden showed that the main influence factors of PICH inpatients were age, GCS score on admission, length of stay and treatment method. Conclusion In the direct medical cost, the ratio of drug fee is too high, and the age of patients, disease severity, average length of stay, and selection of treatment method are the important influence factors of direct economic burdens.
