血清及滑液白介素-17水平与膝骨关节炎退变及膝痛程度的相关性研究 被引量:13

摘要 目的 探讨白介素-17(IL-17)水平与膝骨关节炎(KOA)影像学分期及关节疼痛程度之间的相关性。方法 选取2013年1月-2016年1月于沧州市中心医院就诊的120例KOA患者和30例健康对照者作为研究对象,分别作为实验组和对照组。采用Kellgren-Lawrence分级标准(KL分级)评价实验组膝关节X线影像严重程度。采用视觉模拟(VAS)疼痛评分评价实验组的膝关节疼痛程度。采用酶联免疫法测定两组血清IL-17水平和实验组滑液IL-17水平。分析KOA患者血清及滑液IL-17水平与影像严重程度及膝痛程度之间的相关性。结果 实验组血清IL-17水平显著高于对照组(t=11.611,P=0.000);实验组滑液IL-17水平显著高于其血清水平(t=6.445,P=0.000)。血清和滑液IL-17水平在不同KL分级之间比较,差异无统计学意义(P〉0.05);VAS疼痛评分在不同KL分级之间比较,差异无统计学意义(P〉0.05)。血清和滑液IL-17水平与KL分级之间无相关性(P〉0.05)。KL分级与VAS疼痛评分之间无相关性(P〉0.05)。血清及滑液IL-17水平与VAS疼痛评分均呈显著正相关(r血清·VAS=0.230,P=0.011;r滑液·VAS=0.229,P=0.012)。血清与滑液IL-17水平呈显著相关(r=0.930,P=0.000)。结论 IL-17可作为一种生物学标志物反映KOA患者的疼痛程度,但不能反映疾病的影像严重程度。 Objective To investigate the relationship between interleukin-17 (IL-17) levels in serum and synovial fluid aml the degree of joint degeneration and gonalgia of patients with knee osteoarthritis (KOA). Methods From January 2013 to January 2016, in the Central Hospital of Cangzhou, 120 patients with KOA and 30 healthy individuals were selee, ted aml divided into experimental group and control group. The X-ray image severity of knee joint of patients with KOA was evaluated by the Kellgren-Lawrenee (KL) grades. The degree of gonalgia of patients with KOA was evaluated by visual analog scale (VAS) pain score. The IL-17 levels in serum of two groups and the IL-17 levels in synovial fluid of experimental group were determined by enzyme-linked immunosorbent assay. The con'elation between IL-17 levels in serum and synovial fluid and the degree of joint degeneration and gonalgia of patients with KOA was analyzed. Re- sults The serum IL-17 levels of experimental group were higher than that of control group (t=l 1.611, P=0.000). In experimental group, the IL-17 levels in synovial fluid were significant higher than that in serum (t=6.445, P=0.000). There were no statistically significant differences tot IL-17 levels in synovial fluid and serum and VAS pain scoies among different KL grades (P 〉 0.05). There was no correlation of VAS pain score and the KL grades (P 〉 0.05). There was no correlation of II,-17 levels in serum and synovial fluid with the KL grades (P 〉 0.05). Serum and synovial fluid IL-17 levels were positively correlated with VAS pain score (r plasnt VAS=0.230, P=0.011 ;r synovial fluid.VAS=0.229, P=0.012). Serum fluid 11717 levels showed a positive correlation with synovial fluid IL-17 levels (r=0.930, P=0.000). Conclusion IL-17 is a useful biochemical marker to reflect the degree of gonalgia of the patients with KOA. However, IL-17 can not reflect degenerative severity of KOA in imageology.
