超声联合腹腔镜射频消融术对肝癌患者肝功能及免疫功能的影响

Effect of ultrasound in combined with laparoscopic radiofrequency ablation on the liver function and immunological function in patients with liver cancer

目的:探究超声联合腹腔镜射频消融术(LRFA)对肝癌患者肝功能及免疫功能的影响。方法:选取我院收治的76例肝癌患者,随机分为治疗组及对照组各38例,治疗组采用超声联合LRFA治疗,对照组采用肝动脉栓塞化疗治疗,对比两组治疗前后肝功能和免疫功能指标的变化。结果:术后两组血清谷草转氨酶(AST)、谷丙转氨酶(ALT)、血清总胆红素(T-BIL)、直接胆红素(D-BIL)较术前显著下降(P<0.05),且术后两周治疗组下降程度显著低于对照组(P<0.05);术后两组血浆IgG、IgA和IgM水平明显高于术前(P<0.05),两组间比较差异显著(P<0.05);两组术后CD3+、CD4+、CD8+T淋巴细胞及NK细胞水平较术前均显著上升(P<0.05),CD4+/CD8+较术前有明显下降(P<0.05),且组间差异显著(P<0.05)。结论:超声联合LRFA治疗肝癌能有效保护患者的肝功能及免疫功能,可作为优选方案。

Objective： To explore the effects of ultrasound in combined with laparoscopic radiofrequency ablation （LR- FA） on the liver function and immunological function in patients with liver cancer. Methods.- A total of 76 patients with liver cancer who were admitted in our hospital were included in the study and randomized into the treatment group and the control group with 38 cases in each group. The patients in the treatment group were given ultrasound in combined with LRFA, while the patients in the control group were given TACE. The changes of liver function and immunological function indicators before and after treatment in the two groups were compared. Resul：~ The serum AST, ALT, T-BIL, and D-BIL after operation in the two groups were significantly reduced when compared with before operation （P〈0.05）; moreover, the reduced degree in the treatment group was significantly lower than that in the control group （P 〈0.05）. The plasma IgG, IgA, and IgM levels after operation in the two groups were significantly elevated when compared with before operation （P〈0.05）, and the compar- ison between the two groups was statistically significant （P〈0.05）. CD3＋ , CD4＋ , CD8＋ , and NK levels after operation in the two groups were significantly elevated when compared with before operation （P 〈0.05）, while CD4＋/CD8＋ was signifi- cantly reduced when compared with before operation （P〈0.05） ;moreover, the comparison between the two groups was sta-tistically significant （P〈0.05）. Conclusions： Ultrasound in combined with LRFA in the treatment of liver cancer can protect the liver function and immunological function; therefore, it can be served as a preferred scheme.
