探讨腹腔镜和开腹手术治疗异位妊娠的疗效和生存质量差异

Curative effect laparoscopic and open surgery in treatment of ectopic pregnancy and its difference in quality of life

目的研究并比较腹腔镜手术、开腹手术治疗异位妊娠的疗效及生存质量。方法于2012年1月～2015年12月，选取我院收治的100例异位妊娠患者作为此次研究的对象，采取计算机随机抽取法将患者分为对照组、观察组，每组50例。对照组采取开腹手术治疗，观察组采取腹腔镜手术治疗，比较两组患者的疗效和生存质量评分。结果两组患者的手术时间比较，差异无统计学意义（P〉0．05），但与对照组相比，观察组的术中出血量明显更少（P〈0．05），其术后下床活动时间、肛门排气恢复时间、住院时间均明显缩短（P〈0．05），VAS疼痛评分明显更低（P〈0．05）。观察组患者手术后未出现并发症，对照组共出现2例并发症，两组相比，差异无统计学意义（P〉0．05）。与对照组相比，观察组的生存质量评分明显更高（P〈0．05）。结论在异位妊娠的临床治疗中，实施腹腔镜手术治疗的疗效显著，可有效改善生存质量，且具有较高的安全性。

Objective To study and compare the curative effect of laparoseopic surgery and open surgery in the treatment of ectopic pregnancy and its difference in quality of life. Methods 100 cases of ectopic pregnancy patients cured in our hospital from January 2012 to December 2015 were selected as the research object. According to the computer random sampling method, they were divided into control group and observation group with 50 cases in each. Patients in control group were treated with open surgery, and patients in observation group were treated with laparoscopic surgery. Curative effect and the survival quality score of the two groups were compared. Results There was no significant difference on the time of surgery （P〉 0.05）. But compared with control group, the operation bleeding of observation group was significantly less （P 〈 0.05）, he postoperative bed activity time, anal exhaust recovery time, hospitalization time were significantly shortened （P 〈 0.05）, and VAS pain score was significantly lower （P〈0.05）. There was no complications occurred in observation group, the control group had 2 cases of complications, there was no significant difference between the two groups （P〉0.05）. Compared with control group, the survival quality score of observation group was significantly higher （P 〈 0.05）. Conclusion In the clinical treatment of ectopic pregnancy, the effect of laparoscopic surgery is significant. It can effective!y improve the survival quality, and it has high safety.
