精子相关抗原9(SPAG9)在乳腺癌中的表达及其临床意义 被引量:3

摘要 目的研究乳腺癌患者组织中精子相关抗原9(spermassociated antigen9,SPAG9)和血清中SPAG9抗体的表达水平,了解其对乳腺癌诊断的价值。方法免疫组化(immunohistochemistry,IHC)法检测40例乳腺癌患者病灶及其癌旁组织中SPAG9表达水平,分析SPAG9表达水平与临床病理分期(tumor node metastasisstage,TNM)的关系。酶联免疫吸附试验(enzyme-linked immunosorbent assay,ELISA)检测53例初诊乳腺癌、48例良性病变和43例健康对照血清SPAG9抗体浓度,分析血清SPAG9抗体浓度与患者临床指标的关系;并绘制受试者工作特征(receiver operating characteristic,ROC)曲线,评价血清SPAG9抗体对乳腺癌诊断的价值及意义。结果乳腺癌病灶组织中SPAG9蛋白表达高于癌旁组织,二者差异有统计学意义(P=0.000)。乳腺癌血清SPAG9抗体浓度[10.22(5.055~16.660)mU/m1]显著高于良性病变组[6.666(3.369~11.517)mU/m1](P〈0.05)和健康对照组[5.079(2.857~8.571)mU/m1](P=0.000),且与年龄和TNM分期明显有关。ROC曲线下面积(areaunder the curve,AUC)为0.715,得出SPAG9抗体最佳临界值为10.185mU/ml时,SPAG9抗体诊断乳腺癌的敏感度为50.9%,特异性为88.4%。结论SPAG9及其抗体在乳腺癌病灶组织和血清中高表达,血清SPAG9抗体对乳腺癌诊断具有一定的价值。 Objective To investigate the expression of SPAG9 in breast cancer tissues and level of serum SPAG9 antibody, and e- valuate its diagnostic value for breast cancer. Methods The expression of SPAG9 was detected in 40 paired of breast cancer lesions and matched adjacent tissues by IHC, and the correlation between SPAG9 expression and TNM stages was analyzed. Expression level of SPAG9 antibody was detected in serum of 53 breast cancer patients, 48 patients with benign lesions and 43 normal controls by ELISA, and the correlation between SPAG9 expression and clinicopathological index was analyzed. The diagnostic value of serum SPAG9 antibody in breast cancer was evaluated by ROC curve. Results IHC results showed that high SPAG9 expression was present in breast cancer lesion but low in corresponding adjacent tissues, and the difference was statistically significant (P = 0. 000). Serum SPAG9 antibody level was significantly higher in breast cancer patients [ 10.22 ( 5. 055 - 16. 660) mU/ml ] than in patients with benign lesions [ 6. 666 ( 3. 369 - 11.517 ) mU/ml ] ( P 〈 0.05 ) and normal controls [ 5. 079 (2. 857 - 8.571 ) mU/ml ] ( P = 0. 000), and was closely related with age and TNM stages. ROC curve demonstrated that the AUC was 0. 715, and the best cut -off point of SPAG9 antibody was 10. 185mU/ml. The sensitivity of SPAG9 antibody in breast cancer diagnosis was 50.9% and specificity was 88.4%. Conclusion SPAG9 and its antibody are highly expressed in breast cancer tissues and serum, and the serum SPAG9 antibody has a certain value for breast cancer diagnosis.
