阻塞性睡眠呼吸暂停综合征患者血清IL-6水平与认知功能障碍的相关性研究 被引量:9

Study of correlation of serum IL-6 levels and cognitive dysfunction among patients with obstructive sleep apnoea syndrome

摘要 目的探讨阻塞性睡眠呼吸暂停综合征(OSAS)患者血清白介素-6(ILM)水平与认知功能障碍的相关性。方法通过蒙特利尔认知评估(MoCA)量表对经多导睡眠图(PSG)确诊为OS-AS的102例患者进行认知功能评估,根据评估结果将患者分为不伴认知功能障碍组(n=47例)和伴认知功能障碍组(n=55例);30例健康自愿者作为对照。采用酶联免疫吸附法(ELISA)检测所有对象血清IL-6水平;根据睡眠呼吸暂停低通气指数(AHI)将伴认知功能障碍OSAS患者分为轻(n=15)、中(n=16)、重度(n=24),分析不同程度OSAS患者的MoCA评分及IL6之间的差别;将OSAS伴认知功能障碍患者血清IL-6水平与患者MoCA评分进行相关性分析。结果OSAS伴认知功能障碍患者血清IL-6水平[(197.3±72.58)ps/m1]显著高于不伴认知功能障碍患者[(155±39.3)pg/Ⅱd]及健康对照[(87.39±33.44)ps/ml],差异有统计学意义(P〈0.01)。伴认知功能障碍的OSAS患者,随着病情的加重,MoCA评分逐渐降低,血清IL-6水平逐渐升高(P〈0.01);Pearson相关性分析结果显示:伴认知功能障碍的OSAS患者血清IL-6水平与MoCA评分呈负相关(r=-0.65,P〈0.01)。结论OSAS患者血清IL-6水平在伴认知功能障碍患者中显著升高;伴认知功能障碍的0s.AS患者的AIH分度越高,认知功能障碍越严重,血清IL-6水平越高;血清IL-6水平与患者认知功能障碍程度呈正相关。 Objective To investigate the relationship of serum IL-6 levels with cognitive dysfunc- tion among patients with obstructive sleep apnoea syndrome (OSAS). Methods Patients diagnosed with OSAS by polysomnography (PSG) received assessment of cognitive function with Montreal Congnitive As- sessment (MoCA). According to the result of MoCA assessment, patients were divided into two groups, without cognitive dysfunction group ( n = 47 ) and with cognitive dysfunction group ( n = 55 ) ; 30 health vol-unteers were used as control Serum IL-6 levels were detected by enzyme-linked immunosorbent assay (ELISA) in three groups; according to the apnea hypopnea index (AHI), OSAS patients with cognitive dysfunction were divided into three groups, light group ( n = 15 ), moderate group ( n = 16 ), and severe group (n = 24) , and differences in MoCA scores and serum 1L-6 levels were analyzed among three groups. The correlation of serum IL-6 levels and MoCA scores were analyzed in OSAS patients with cognitive dys- function. Results Serum levels of IL-6 in OSAS patients with cognitive dysfunction [ ( 197. 3±72. 58 ) pg/ ml] were significantly increased than those without cognitive dysfunction [ (155±39. 3)pg/ml] as well as health control [ (87. 39±33.44)pg/ml] (P 〈0.01). For OSAS patients with cognitive dysfunction, Mo- CA scores were decreased and the level of serum IL-6 was increased among patients with poor AHI grade, with statistically significant difference. Pearson correlation analysis showed serum IL-6 levels were significantly negatively correlated with MoCA scores among OSAS patients with cognitive dysfunction (r = - 0. 65, P 〈 0. 01 ). Conclusions Serum levels of IL-6 in OSAS patients with cognitive dysfunction were significandy increased and correlated with the cognitive dysfunction. OSAS patients with cognitive dysfunction experiencing sever AHI grade have more Serious cognitive dysfunction and the higher serum levels of IL-6.
