红元胶囊对老年髋部骨折术后凝血及骨代谢指标的影响

Effect of Hongyuan capsule on indexes of coagulation and bone metabolism in elderly patients with hip fracture

目的探讨红元胶囊对老年患者髋部骨折术后凝血及骨代谢指标的影响。方法老年髋部骨折60例,均为股骨粗隆间骨折,将患者随机分为试验组(30例)和对照组(30例)。试验组口服红元胶囊3粒,3次/d;对照组行低分子肝素钙皮下注射。两组患者同时用药至术后35 d。观察两组患者的凝血功能[凝血酶原时间(PT)、活化部分凝血酶原时间(APTT)、纤维蛋白原含量(FIB)、D-二聚体含量(DDI)]、骨质疏松三项(TPINP、β-Crosslaps、N-MID)和肿胀及疼痛消失时间。结果两组患者FIB、DDI较术前均降低,而APTT和PT与手术前比较均延长,但两组用药后比较差异无统计学意义(P>0.05)。两组患者骨质疏松三项均升高(P<0.01);试验组患肢肿胀程度、缩短疼痛持续时间方面优于对照组,差异有统计学意义(P<0.01)。两组预防下肢深静脉血栓效果比较,差异无统计学意义(P>0.05)。结论红元胶囊可以防治老年髋部骨折术后深静脉血栓,对骨折愈合起到积极作用,同时能有效改善患者术后并发症。

Objective To explore the effect of hongyuan capsule on coagulation and bone metabolic indexes of elderly patients after hip fracture.Methods Sixty elderly patients with intertrochanteric fracture were randomly divided into experiment group（n =30） and control group（n =30）.Experiment group was given three Hongyuan capsules,3times daily,while control group was treated with low molecular weight heparin calcium.All patients were treated for 35 d after operation.The serum levels of PT,APTT,FIB,DDI,T-PINP,β-Crosslaps,N-MTD,and the relieving of swelling and pain of injured thighs were assessed.Results The serum FIB and DDI levels in the two groups decreased after treatment,and serum PT and APTT were prolonged,but there was no significant difference between the two groups after treatment（P0.01）.After treatment,T-PINP,β-Crosslaps and N-MID in both groups were elevated（P〈0.05）.However,the status of relieving of swelling and pain of injured thighs in treatment group was better than that of control group（P〈0.05）.The postoperative incidences of DVT in both groups showed no significant difference.Conclusion Hongyuan capsule can prevent DVT,promote fracture healing,and can improve the postoperative complications effectively in elderly patients of hip fracture.
