2009-2013年山东省六县区主要恶性肿瘤的死亡状况及生命表分析

Death Status and Life Table Analysis of Malignant Tumor in Six Counties of Shandong Province from 2009 to 2013

目的了解山东省2009-2013年六县区恶性肿瘤的死亡状况及疾病负担。方法利用山东省2009-2013年6个监测地区的恶性肿瘤死亡资料,采用粗死亡率、标化死亡率及单递减原因生命表,探讨恶性肿瘤及其主要类型对预期寿命的影响。结果山东省恶性肿瘤死亡率由2009年的163.4/10万上升至2013年的178.5/10万,各年份间差异有统计学意义,男性恶性肿瘤死亡率高于女性;2009-2013年,山东省6个监测地区前12位的恶性肿瘤死因顺位变化较小,女性生殖系统恶性肿瘤的死因顺位变化相对较大,肺癌、肝癌、胃癌、食管癌及结直肠癌居于总死因顺位的前5位;2009-2013年山东省居民的去死因预期寿命呈上升趋势,但期望寿命增长值整体呈现下降趋势;去恶性肿瘤对各年龄段男性预期寿命的影响高于女性。结论恶性肿瘤是影响山东省居民健康和经济发展的主要健康因素,虽然标化死亡率指标呈下降趋势,但由于人口老龄化加剧,粗死亡率并未表现出下降趋势,仍需保持预警。

Objective To evaluate the burden of disease of cancer in Shandong Province from 2009 to 2013. Methods Use the death data of malignant tumor of the residents in 6 surveillance sites from 2009 to 2013 in Shandong Province, the in- fluence of the malignant tumor and its main types on the life expectancy was discussed by using the life table of crude mortality, standardized mortality rate and single decrement. Results The malignant tumor mortality is rising from 163.4/100000 in 2009 to 178.5/100000 in 2013. The difference of each year was statistically significant. The crude death rate and standardized mortali- ty rate of male is higher than that of female;The change of the death of the first 12 malignant tumor is small from 2009 to 2013, but the female reproductive system malignant tumor is relatively large. The top 5 of the death causes is lung cancer,liver cancer, gastric cancer, esophageal cancer and colon cancer;The life expectancy of residents in Shandong province is increasing, but the o- verall trend of life expectancy is declining from 2009 to 2013 ;The influence of the cancer to the life expectancy of the malignant tumor was significantly higher than that of the female. Conclusion Malignant tumor is the main factor that affects the health and economic development of Shandong province. Although the standardized mortality index shows a downward trend, but the crude mortality has not shown a downward trend due to the aging population.
