协同护理模式对乳腺癌根治术病人生存质量及化疗副反应的干预探讨

Influence of collaborative care mode on quality of life and chemotherapy side effects in patients undertakingradical mastectomy

目的通过本研究探讨针对乳腺癌根治术病人采取协同护理模式对其生存质量及化疗副反应的影响。方法研究对象选取自本院2013年6月至2016年3月期间收治的乳腺癌根治术后化疗病人72例，按照数字分组法将入选对象分为干预组与对照组，每组36例；对照组病人给予常规护理，干预组病人在常规护理的基础上实施协同护理模式，对两组病人疼痛、毒副反应情况及生活质量评分进行比较。结果对干预组与对照组病人分别实施协同护理模式和常规护理模式后，干预组的无痛率达到22．22％，而对照组中的无痛率仅为2．78％，干预组疼痛程度明显轻于对照组；毒副反应方面干预组患者的I级、Ⅱ级毒副反应明显高于对照组，而在Ⅲ级、Ⅳ级毒副反应明显低于对照组；两组病人出院后SF-36评分比较，干预组明显高于对照组（P〈0．05）。结论对乳腺癌根治术病人实施协同护理模式有利于发挥病人及其家属的能动性作用，可使病人化疗期间副反应减少，并改善患者生活质量，在临床中有重要的价值。

Objective To explore the influence of collaborative care mode on quality of life and chemotherapy side effects in patients undertaking radical mastectomy. Methods 72 patients undertaking chemotherapy after radical mastectomy at our hospital from June, 2013 to March, 2016 were selected as study objects and were divided into an intervention group and a control group using digital packet method, 36 cases for each group. The control group were routinely cared; in addition, the intervention group received collaborative care mode. The pain, adverse reactions, and quality of life were compared between these 2 groups. Results After the nursing care, the painless rate was 22.22% in the intervention group and 2.78% in the control group. The pain degree was milder in the intervention group than in the control group. The incidences of grade I and II adverse side effects were obviously higher while those of grade Ⅲ and IV lower in the intervention group than in the control group. The SF-36 scores were significantly higher the intervention group than in the control group （P 〈0.05）. Conclusions Collaborative care mode for patients undertaking radical mastectomy is good in improving the patients＇ and their family members＇ initiative and can reduce the side effects of chemotherapy and improve the patients＇ quality of life, so it has important clinical value.
