肺源性心脏病患者血清sST2、NT-proBNP、超声心动图参数的变化特征及相互关系 被引量:22

摘要 目的探讨血清可溶性人基质裂解素2(solublehumanstromelysin-2,sST2)对肺源性心脏病(以下简称肺心病)的诊断价值。方法选取2016年1月1日至2016年3月31日期间我科诊治的肺心病患者57例作为研究组,根据氮末端脑钠肽前体(N-terminalpro-brainnatriureticpeptide,NT-proBNP)分为:A1组27例(NT-proBNP<4000pg·mL^(-1))、A2组30例(NT-proBNP≥4000pg·mL^(-1)),以同期我院21例健康体检者作为对照组(N组),所有受试者年龄≥45岁,组间年龄、性别无差异(P>0.05)。所有受试者完成经胸超声心动图检查,并采用双抗夹心ELISA法定量检测静脉血清sST2浓度,分析sST2、NT-proBNP、超声心动图参数的变化特征及相互关系,探讨sST2对肺心病的诊断价值。结果(1)A2组sST2高于N组和A1组,差异有统计学意义,P<0.05;(2)sST2和NT-proBNP呈正相关,相关系数r=0.538,P<0.05;(3)sST2和右室内径、右室流出道内径、肺动脉压呈正相关,相关系数r=(0.513,0.492,0.421),P<0.05.结论sST2可作为肺心病诊断的生物学标志物。 Objective To investigate the diagnostic value of serum sST2 in pulmonary-heart disease patients by analyzing the relationship and characteristics among SsrI'2, NT-proBNP and echocardiography parameter. Methods 57 patients with chronic pulmonary-heart disease from January 1st to March 31st, 2016, were selected as the pulmonary-heart disease group, and they were divided into 2 groups according to NT-proBN : 27 in the group A1 ( NT- proBNP 〈4000 pg · mL-1 ) and 30 in the group A2 ( NT-proBNP≥4000pg · mL-1 ). Meanwhile, 21 healthy individuals were taken as the control group ( group N). All subjects were older than 44 years, and there was no difference among these groups in age and gender. The level of serum sST2 was tested by ELISA and the echocarcliography parameters were collected. The relationships among sST2, NT-proBN and echocardiography parameters in the 3 groups were evaluated. Results 1. The level of serum sST2 was significantly higher in the group A2, than in the group N and A1 ( P 〈 0. 05 ). 2. Pearson correlation analysis showed that the correlation coefficient was 0. 538 (P 〈 0. 05 ) when relationship was analyzed between sST2 and NT-proBNP. 3. There was positive correlation among sST2 and RVDd, RVOT or PPA, and the correlation coefficient was 0. 513, 0. 492, and 0. 421, respectively (P 〈0. 05). Condusion sST2 can be used as an efficient biomarker of pulmonary-heart disease.
