摘要
目的探讨呼吸机环路给氧应用于急性呼吸衰竭机械通气患者的临床疗效及其安全性。方法选取2013年2月至2015年8月我院收治的急性呼吸衰竭患者80例,采取随机数字表法分为观察组与对照组,在常规治疗的基础上,观察组患者给予呼吸机环路给氧,对照组患者给以常规的氧源供氧,对比两组患者治疗前后呼吸频率、心率、血气分析各项指标的变化,并对比两组患者于呼吸机相关肺损伤的发生率。结果与治疗前对比,观察组与对照组患者的心率及呼吸频率在治疗后1-2h及12-24h均显著改善,差异有统计学意义(P<0.05),但两组间在治疗前后心率及呼吸频率均无显著差异,差异无统计学意义(P>0.05);与治疗前对比,观察组与对照组患者的pH值、PaCO_2及PaO_2均显著改善,差异有统计学意义(P<0.05),但两组间治疗前后pH值、PaCO_2及PaO_2无显著差异,差异无统计学意义(P>0.05);观察组与对照组患者在通气期间均未出现与呼吸相关的肺损伤及其他并发症。结论在给氧设备不完善时,为抢救危重症患者,可以进行呼吸机环路给氧。
Objective To investigate the safety and efficacy of ventilator loop oxygen for mechanical ventilation in patients with acute respiratory failure. Methods 80 patients with acute respiratory failure from February 2013 to August 2015 were randomly divided into the observation group and the control group. Based on conventional treatment, the observation group was given ventilator loop oxygen, and the control group was given regular source of oxygen. Their respiratory frequency, heart rate, blood gas analysis, and ventilator associated lung injury incidence were compared between the two groups. Results 1 -2h and 12 -24h after treatment, their heart rate and respiratory rate improved significantly (P 〈 0. 05) in the two groups, but it showed no significant difference between the two groups ( P 〉 0. 05 ). The levels of pH, PaCO2 and PaO2 improved significantly in the two groups ( P 〈 0. 05 ), but it showed no significant difference between the two groups (P 〉 0.05). There was no respiratory related lung injury and other complication occurred during ventilation. Conclusion When the oxygen supply equipment is not perfect, ventilator loop oxygen can be used to save the critically ill patients.
