体内胰岛素水平与子宫内膜癌发生关系的探讨

Relationship between insulin levels and endometrial carcinoma

目的探讨体内胰岛素水平与子宫内膜癌发生发展的关系。方法选取48例子宫内膜癌患者(研究组)和50例健康查体的女性(对照组),观察记录两组患者的临床指标,包括年龄、绝经年龄、体质量、腰围、月经史、孕产史、既往史(糖尿病、高血压等)、类固醇激素的使用情况等,并检测两组患者空腹血清C肽、胰岛素、雌二醇、雄烯二酮、雌酮、睾酮及性激素结合球蛋白(SHBG)水平。结果研究组的空腹血清C肽、胰岛素、雌酮、雌二醇、雄烯二酮及睾酮水平均高于对照组(P﹤0.05),SHBG水平低于对照组(P﹤0.05)。血清C肽及胰岛素检测水平与雌酮、雌二醇、雄烯二酮及睾酮水平呈正相关,与SHBG水平呈负相关。结论子宫内膜癌发病的原因可能是胰岛素对体内性激素产生负面影响。

Objective To investigate the correlation between insulin levels and the occurrence and development of endometrial carcinoma. Method 48 patients with endometrial carcinoma and 50 cases of healthy female adults were enrolled in the study as study group and control group, respectively, of which the clinical characteristics were recorded, including age, time of menopause, weight and waist circumference, menstrual history, maternal history, prior medical history（diabetes, high blood pressure, etc.）, the usage of steroid hormone, and so on, the fasting serum C- peptide, insulin（INS）, estradiol（E2）, androstenedione（A）, estrone（E1）, testosterone（T） and sex hormone binding globulin（SHBG） levels were detected in both groups. Result The levels of fasting serum C-peptide, INS, E1, E2, A and T were significantly higher than that of control group（P〈0.05）, but SHBG level was lower compared with the control group（P〈0.05）. The levels of fasting serum C-peptide and INS were positively correlated with E2, A and T, while conversely associated with SHBG level. Conclusion The occurrence of endometrial cancer may be attributed to the negative effect that insulin exerts on sex hormones in the body.
