摘要
目的:探讨采用Acutrak螺钉治疗儿童胫骨结节骨折的临床疗效。方法:选择本院2010年10月~2015年10月收治的儿童胫骨结节骨折37例,折端间隙均>2mm,根据Salter-Harris分型标准,Ⅱ型20例,Ⅲ型17例。随机分成手术组19例,对照组18例,手术组均采用切开复位Acutrak螺钉内固定治疗,对照组采用手法复位石膏外固定。均采用Lysholm膝关节功能评分标准进行疗效评定。并对手术组及对照组膝关节功能恢复情况进行比较,用SPSS18.0软件进行统计学分析,采用x^2检验。结果:37例患儿均获得随访,时间12~36个月,平均24.7个月。其中手术组优17例,良2例,对照组优16例,良2例。但是对照组出现关节功能恢复慢甚至僵硬,较手术组明显增多(P<0.05)。结论:采用切开复位Acutrak螺钉内固定治疗儿童胫骨结节骨折(Ⅱ型和Ⅲ型)取得满意疗效,具有可行性。
Objective:To investigate the clinical effect of Acutrak screw in the treatment of children with tibial adolescent fractures. Methods:From Octorbor 2010 to Octorbor 2015, we select 37 cases of tibial adolescent fractures in children in our hospital with Fracture gap>2 mm.According to Salter-Harris'criterion of trauma type, typeⅡ20 cases,typeⅢ17 cases.19 cases were randomly divided into operation group and 18 cases in control group, The surgical group were treated with open reduction and internal fixation with Acutrak screws, the control group was treated with manipulative reduction and plaster external fixation.Lysholm knee joint function score was used to evaluate the efficacy of the knee joint function score.The surgical group and control group in the recovery of knee joint function were compared using SPSS18.0 software for statistical analysis with χ2 test.17 cases were excellent and 2 cases were good in the surgical group, 16 cases were excellent and 2 cases were good in the control group.Results:37 patients were received follow up .The average pe-riod was 24.7 months(range 12~36 months).17 cases were excellent and 2 cases were good in the surgical group, 16 cases were excel-lent and 2 cases were good in the control group.However, the control group showed that the recovery of joint function was slow and even more rigid, more than the operation group(P〈0.05).Conclusion:Adoping Open reduction and Acutrak screw fixation to treat the frac-ture of proximal tibia in adolescents ( TypeⅡand typeⅢ) will get the good therapeutic effects and is feasible.
