锁定钢板内固定结合同种异体骨植骨治疗复杂胫骨平台骨折的疗效观察

Curative effect observation of locking plate internal fixation combined with allograft bone graft in the treatment of complex tibial plateau fracture

目的分析探讨在复杂胫骨平台骨折患者使用锁定钢板内固定结合同种异体骨植骨治疗的临床疗效。方法选取2014年12月至2016年6月就诊于我院的78例复杂胫骨平台骨折患者为研究对象。根据治疗方法的不同,随机分为干预组和常规组,每组39例患者。常规组采用锁定钢板内固定方法进行治疗,干预组使用锁定钢板内固定结合同种异体骨植骨方法进行治疗。比较不同方法治疗后,两组患者的关节功能评分情况以及手术后患者关节下沉发生率等情况。结果干预组患者的关节功能HSS评分优良率为94.87%,明显高于常规组患者的87.18%,组间比较,差异具有统计学意义(P<0.05),治疗后干预组患者的关节下沉率为0.00%,明显低于常规组患者的10.26%,组间比较,差异具有统计学意义(P<0.05)。结论在复杂胫骨平台骨折患者中使用锁定钢板内固定结合同种异体骨植骨治疗的临床疗效较好,关节功能评分较高,治疗后患者的并发症发生率较低,在临床上应积极地予以应用与推广。

Objective To investigate the clinical efficacy of locking plate fixation combined with allograft bone graft in the treatment of complex tibial plateau fractures. Methods Seventy-eight cases of patients with complex tibial plateau fractures in our hospital from December 2014 to June 2016 were selected as the study objects. And all the patients were randomly divided into intervention group （39 cases） and conventional group （39 cases） according to different treatment methods. The conventional group was treated with locking plate internal fixation method, and the intervention group was given locking plate fixation combined with allograft bone graft method. After treating with different methods, the joint function score and sink rate between the two groups were recorded and compared. Results The HSS score of joint function in the intervention group was 94.87%, which was higher than 87.18% in the conventional group, and the difference between the two groups was statistically significant （P〈0.05）. After treatment, the rate of subsidence of joints in the intervention group was 0.00%, which was remarkablely less than 10.26% in the conventional group, the difference between the two groups was statistically significant （P〈0.05）. Conclusion Locking plate internal fixation combined with allograft bone graft in the treatment of complex tibial plateau fracture has a better clinical curative effect, higher joint function score and lower complication rate. It should be popularized and applied in clinic.
