摘要
目的探讨急性生理和慢性健康状态评分系统(APACHEⅡ)在评估慢性阻塞性肺疾病急性加重期(AECOPD)合并呼吸衰竭患者护理风险及干预中的应用效果。方法选取呼吸内科重症监护室(RICU)的AECOPD合并呼吸衰竭住院患者84例,随机分为观察组和对照组,对照组给予常规护理,观察组根据患者APACHEⅡ评分结果实施小组护理干预,比较2组患者临床治疗效果、病死率现状、住院时间、护理质量和满意度。结果观察组患者的病死率、并发症发生率显著低于对照组(P<0.05);观察组患者ICU入住时间、机械通气时间、平均住院时间均短于对照组(P<0.05),患者满意度高于对照组(P<0.05)。结论依据AECOPD合并呼吸衰竭患者的APACHEⅡ评分分析护理风险并实施小组护理干预,可以有效降低护理风险,减少并发症的发生,降低患者病死率,缩短了临床机械通气时间,减少患者平均住院日,提高了护理质量和服务满意度。
Objective To explore the application effect of acute physiology and chronic health rating system（ APACHE II） scoring system in evaluating the nursing risk of patients with acute exacerbation of chronic obstructive pulmonary disease（ AECOPD） complicated with respiratory failure. Methods A total of 84 AECOPD patients with respiratory failure in our hospital in respiratory intensive care unit（ RICU） were randomly divided into observation group and control group. The control group was given routine nursing according to clinical symptoms and subjective judgments,while the observation group was given nursing intervention according to APACHE II score. Clinical curative effect,mortality rate,hospitalization time,nursing quality and satisfaction were analyzed. Results The mortality rate and complications in the observation group were significantly lower than the control group（ P 〈 0. 05）. Duration of ICU stay,mechanical ventilation time and average hospitalization time in the observation group were shorter and satisfaction was significantly higher than the control group,and the difference was statistically significant（ P 〈 0. 05）. Conclusion Nursing risk and nursing risk analysis for AECOPD patients with respiratory failure can effectively reduce the risk of nursing,reduce complications and mortality rate,shorten time of mechanical ventilation and the average daysof hospitalization,and improve the quality of nursing and service satisfaction,so it is worthy of clinical promotion.
