左旋甲状腺素联合硒酵母对桥本甲状腺炎患者galectin-3表达及甲状腺功能的影响

The influence of levothyroxine combined with selenium yeast on galectin-3 expression and thyroid function in the treatment of thyroiditis

目的：探讨左旋甲状腺素联合硒酵母对桥本甲状腺炎患者galectin-3表达及甲状腺功能的影响。方法：选择我院于2014年1月至2015年11月期间收治的106例接受手术治疗的桥本甲状腺炎患者作为研究对象,将所有患者随机分为观察组和对照组,每组各53例。对照组行口服左旋甲状腺素片治疗,观察组患者在对照组治疗方案基础上予以口服硒酵母联合治疗,对两组患者的临床治疗效果和治疗前后FT3、FT4、TSH、TG-Ab、TPO-Ab水平和galectin-3表达情况以及不良反应发生率等进行比较观察。结果：观察组FT3和FT4水平显著高于对照组,TSH、TG-Ab和TPO-Ab水平显著低于对照组,差异有统计学意义（P〈0.05）;两组患者治疗后的galactin-3表达阳性率和染色指数均显著低于治疗前,且观察组治疗后的galactin-3表达阳性率和染色指数明显低于对照组,差异有统计学意义（P〈0.05）;两组患者治疗过程中均无明显不良反应发生,差异无统计学意义（χ2=0.05,P=0.99）。结论：采用左旋甲状腺素联合硒酵母治疗桥本甲状腺炎疗效确切,能够显著降低galectin-3表达,改善患者甲状腺功能和免疫功能,值得临床推广应用。

Objective： To explore the influence of levothyroxine（ LT4） combined with selenium yeast on galectin-3 expression and thyroid function in the treatment of thyroiditis. Methods： 106 patients with thyroiditis from January 2014 to November 2015 in our hospital were selected as the research objects,all of them were randomly divided into control group and study group,the control group（ 53 cases） received LT4,the study group（ 53 cases）received LT4 combined with selenium yeast. The clinical efficiency,thyroid function（ FT3,FT4,TSH）,the levels of thyroid autoantibodies（ TG-Ab,TPO-Ab）,Galectin-3 expression and adverse effects rate between the two groups were compared and analyzed. Results： The levels of FT3 and FT4 of the study group were significantly higher than those of the control group and the levels of TSH,TG-Ab and TPO-Ab of the study group were lower,the differences were statistically significant（ P 〈 0. 05）. After the treatment,the positive frequency and stain index of galectin-3 had decreased in both groups,the differences were statistically significant（ P 〈 0. 05）; the positive frequency of galectin-3 expression and stain index of the study group were significantly lower than those of the control group（ P 〈 0. 05）. There was no difference on the adverse effects rate between the two groups（ P 〉 0. 05）.Conclusion： LT4 combined with selenium yeast in the treatment of thyroiditis is significantly effective,it can significantly lower the galectin-3 expression,improve the thyroid function and immunologic function with high safety,worthy of promotion.
