益肾健脾化浊祛瘀行水法干预糖尿病肾病的临床观察

Clinical Observation of Yishen Jianpi Huazhuo Quyu Xingshui Method in Treating Diabetic Nephropathy

目的:观察益肾健脾,化浊祛瘀行水法治疗糖尿病肾病的临床作用。方法:将确诊的40例糖尿病肾病患者随机分为治疗组20例和对照组20例。对照组在一般治疗的基础上常规调脂,降压,胰岛素调糖,治疗组在对照组的基础上加服自拟方平消汤,1个月为1个疗程;比较2组治疗前后FPG,2hPG,HbA1c、BUN、CREA、UAER、SBP、DBP、血脂分析、24h蛋白定量等指标的变化。结果:2组治疗后FPG、2hPG、HbAlc、CHOL、TG、LDL-C、UAER、24h蛋白定量均下降,与对照组比较差异有统计学意义(P<0.05或P<0.01)。治疗组治疗后CREA下降,与对照组比较差异有统计学意义(P<0.05)。2组治疗后BUN、收缩压、舒张压均下降,与对照组比较差异无统计学意义(P>0.05)。2组治疗前后HDL差异均无统计学意义(P>0.05)。结论:自拟方平消汤治疗糖尿病肾病具有一定疗效,具有降糖、调脂,降低血肌酐,减少蛋白尿,改善肾功能,延缓肾损害的发展的作用,改善患者的临床症状。

Objective： To observe the effect of Yishen Jianpi Huazhuo Quyu Xingshui method in treating diabetic nephropathy. Methods： 40 patients with diabetic nephropathy were randomly divided into treatment group （n = 20） and control group （n = 20）. Lipid lowering, anti-hypertensive, insulin was used in the control group based on the general treatment. Self-made Pingxiao decoction was used in the treatment group as addition. One mouth was decide as a treatment course. The changes of FPG, 2hPG, HbAlc, BUN, CREA, UAER, SBP, DBP, blood lipid analysis, 24h protein quantification before and after treatment were compared. Results： The levels of FPG, 2hPG, HbAlc, CHOL, TG, LDLC, UAER, 24h protein in the two groups decreased after treatment, and there was significant difference compared with the control group （P 〈0.05 or P 〈0.01）. Compared with the control group, the difference of CREA between treatment group and control group was statistically significant （P 〈0.05）. The BUN, systolic blood pressure and diastolic blood pressure of the two groups decreased after treatment, and there was no significant difference compared with the control group （P〉 0.05）. There was no significant difference in HDL between the two groups before and after treatment （P〉 0.05）.Conclusion： Self-made Pingxiao decoction has certain curative effect on diabetic nephropathy. It has the effects of low- ering blood sugar, regulating blood fat, reducing serum creatinine, decreasing proteinuria, improving renal function and delaying the development of renal damage and improving the clinical symptoms of diabetic nephropathy.
