脱落细胞学联合癌胚抗原、糖链抗原125、CA15-3、CA19-9检测对恶性浆膜腔积液的诊断价值

Clinical diagnostic value of exfoliocytology examination combined with tumor markers detection including carbohydrate antigen 125,CA15-3,CA19-9 in diagnosis of malignant dropsy of serous cavity

目的研究脱落细胞学联合癌胚抗原(CEA)、糖链抗原125(CA125)、CA15-3、CA19-9检测对恶性浆膜腔积液的诊断价值。方法随机选取160例浆膜腔积液患者,根据良性和恶性表现分为对照组和试验组,每组80例。分析统计患者的CEA、CA125、CA15-3和CA19-9水平。并通过不同的诊断手段对恶性浆膜腔积液患者进行诊断,观察不同诊断手段的诊断性能。结果脱落细胞学联合癌胚抗原、糖链抗原125、CA15-3、CA19-9检测对恶性浆膜腔积液的诊断敏感性为98.2%,特异性为99.1%,准确性97.2%,明显优于其他诊断方法(P<0.05)。结论脱落细胞学联合癌胚抗原、糖链抗原125、CA15-3、CA19-9检测可以显著提高恶性浆膜腔积液的诊断特异性和准确率,值得临床中广泛开展。

Objective To investigate the clinical significance of exfoliocytology examination combined with tumor markers detection including carbohydrate antigen 125, CA15-3, CA19-9 in diagnosis of malignant dropsy of serous cavity . Methods One hundred and sixty patients with malignant dropsy of serous cavity were enrolled in the study .According to the clinical manifestations （ benign or malignant ） these patients were divided into control group and trial group ,with 80 patients in each group.The expression levels of CEA , CA125, CA15-3, CA19-9 were detected and compared between two groups . Moreover the advantages and disadvantages of different diagnosis methods were observed and compared .Results The sensitivity,specificity, accuracy of exfoliocytology examination combined with carbohydrate antigen 125, CA15-3, CA19-9 detection in diagnosis of malignant dropsy of serous cavity were 98.2,99.1%, 97.2%,respectively,which were superior to those by the other diagnostic methods （ P 〈0.05）.Conclusion The exfoliocytology examination combined with carbohydrate antigen 125,CA15-3,CA19-9 detection can obviously increase specificity and accuracy in diagnosis of malignant dropsy of serous cavity,thus,which is worth using widely in clinical practice .
