原发肿瘤切除对晚期乳腺癌患者远期预后的影响

Effect of Primary Tumor Resection on Long Term Prognosis of Patients with Advanced Breast Cancer

目的 观察原发肿瘤切除对晚期乳腺癌患者根治术后远期预后的影响。方法 以2013年6月-2014年7月在本院接受姑息治疗的85例晚期乳腺癌患者为观察对象。按其是否接受原发肿瘤切除分为对照组（未接受过原发肿瘤切除）和观察组（接受过原发肿瘤切除）。观察两组患者1年生存率、复发、转移的发生率。比较两组患者治疗前后肿瘤标志物（CEA、CA15-3和CA125）水平和生活质量的变化。结果 观察组患者一年生存率明显高于对照组,复发和转移的例数明显低于对照组（P〈0.05）;两组患者治疗前CEA、CA15-3和CA125水平无明显差别,治疗后,观察组的上述指标低于对照组（P〈0.05）;两组患者治疗前生活质量各维度得分无明显差别,治疗后,观察组患者得分高于对照组（P〈0.05）。结论 行根治术前曾接受原发肿瘤切除的患者1年生存率较高,复发转移例数较少,可明显提高患者的生活质量。

Objective To observe the effect of primary tumor resection on the long-term prognosis of patients with stage advanced breast cancer. Methods 85 patients with advanced breast cancer treated in our hospital from June 2013 to July 2014 were observed.They was divided into control group （not receive primary tumor resection） and observation group （receive primary tumor resection）.The incidence rate of 1 year survival rate, recurrence and metastasis of the two groups were observed.The level of tumor markers and quality of life were compared between the two groups before and after surgery. Results The survival rate in the observation group patients was significantly higher than that in the control group, the number of cases of recurrence and metastasis was significantly lower than that of the control group, the difference was statistically significant （P〈0.05）.In the two groups of patients before treatment, CEA, CA15-3 and CA125 levels were not significantly different, after treatment ,the observation group was lower than the control group （P〈 0.05 ）. There were no significant differences in the scores of quality of life between the two groups before treatment.After treatment, the patients in the observation group were significantly higher than those in the control group （P〈0. 05）. Conclusion The patients who received resection of the primary tumors had a higher one year survival rate, less recurrence and metastasis of tumor and a better quality of life.
