广东省基层医务人员基层首诊制认知分析

Cognitive analysis of community physicians based on first-treatment system in Guangdong

目的了解广东省基层医务人员对基层首诊制认知和愿望。方法采用整群抽样的方法,对参加广东省家庭医生高峰论坛的基层医务人员175人进行不记名调查,回收有效率为93.14%。结果 1对于基层首诊制的认同,93.3%调查对象赞成建立基层首诊制;253.4%、52.8%的调查对象认为基层首诊的优势是"全科医生全面负责健康"、"方便就诊",仅有5.5%的调查对象认为基层首诊的优势是"医保政策规定"。3所有对象都认为基层首诊制推行的主要障碍集中在基层卫生服务体系不完善、缺乏门诊医保支持和国家对基层投入不足。4对于束缚基层医疗服务能力的原因分析上,研究对象皆认为缺乏辅助检查设备、基药不能满足需求、缺乏完善绩效工资分配制度是主要影响因素。结论基层首诊制的推广建立在将来是一个大概率事件,但以现有条件而言,距离推广基层首诊制还有一定现实差距。通过基层自我完善,自我巩固以及自我强化是实行基层首诊制的根本出路。

Objective To investigate the wishes and cognition of community physicians on the first-treatment system in Guangdong province,bring up the policy suggestions for the establishment and perfection of Community Physicians according to this analysis.Methods With a clustered sampling method,175 community physicians were chosen to complete the questionnaire of the family doctor peak BBS in Guangdong province. Questionnaire recycling effective rate was 93.14%.Results 1 Community esteemed aspects of system,there was 93.3% community medical staff in favor of the establishment of community first-treatment system. 2 There were 53. 4% and 52. 8% of three groups of respondents ragarding community first option system advantage as general physician in charge of health and convenient visit,there was only 5.5% regarding as medical insurance policy. 3The first-treatment system was difficult to implement,three groups agreed that the main reason to focus on community health service system was imperfect,the lack of investment and lack of community.4On the obstacles to the analysis of the causes of community service ability to ascend,three groups of people all thought that the lack of auxiliary examination equipment 〉base medicine could not meet demand〉 the lack of perfect performance salary distribution system is the main influence factors.Conclusions First-treatment system on a large scale set up in the future is a certain event,but in terms of existing conditions,is still some distance to promote grassroots first option system reality gap. Only through self-improvement at the grass-roots level,self to consolidate and strengthen self is the make and the fundamental way out for the first option on first-treatment system.
