颏下区岛状皮瓣在口腔颌面部缺损修复中的应用

Application of the submental artery island flap for repairing the oral and maxillofacial defects

目的：采用颏下区岛状皮瓣修复口腔颌面部恶性肿瘤切除后的组织缺损，探讨其临床价值。方法：选取2013年8月～2016年2月我科收治的9例口腔颌面部肿瘤患者，采用颏下区岛状皮瓣对肿瘤切除后的缺损进行修复，观察术后效果。结果：术后9例修复皮瓣均存活。5例男性患者可见皮瓣表面有不同程度毛发增长，术后3月口内皮瓣向口腔黏膜化生，皮瓣表面光滑、颜色红润。9例患者口腔颌面部的外观及主要功能均达到预期效果，可进行吞咽、咀嚼和言语等正常生理活动，无张口受限、颈部活动受限等并发症。随访期无癌肿复发病例。结论：颏下区岛状皮瓣较其他部位的皮瓣制作相对简单，耗时少，切口隐蔽，对供区损伤小，术后存活率高，可提高患者对术后美观和生理功能的满意度。

Objective：To assess the clinical value of applying submental artery island flap to reconstruction of oral and maxillofacial defects following canc-er ablation.Methods：Nine cases of maxillofacial cancer admitted to our department were included from August 2013 to February.All patients received re-construction of maxillofacial defects with facial submental artery island flap in compliance with the biopsy findings and operative indications as well as sim -ultaneous lymph node resection and postoperative radiotherapy .Results：The flaps were totally survived in 9 cases without necrosis or postoperative infec-tions.Hair growth was seen on the surface of flaps placed in mouth in 5 male patients,yet the flaps grew into mucosa in 3 months after surgery and demon-strated smooth surface and red color.Maxillofacial appearance and physiological functions were well recovered in 9 cases,including normal swallowing,che-wing and speaking.No difficulties of opening mouth and neck movement as well as relapse of tumor occurred during the follow-up periods.Conclusion：Re-construction of the oral and maxillofacial defects with submental island flap is relatively simple in operation compared to other designed flaps .This tech-nique may shorten the operative time,and cause minimal damage to the donor site with cancelled incision ,which can also ensure high survival of the flap, better cosmetic results and oral physiological function after surgery .
