微创保胆取石术与腹腔镜胆囊切除术治疗胆结石的手术效果及对生活质量的影响

目的 研究微创的保胆取石的手术疗法和经过腹腔镜的胆囊切除手术对于胆结石疾病的疗效以及两种手术疗法对病人生活质量的影响.方法 采用随机表法搜集在 2014 年 1 月到 2015 年 12 月于医院诊治的患有胆结石疾病的患者共 150 例,根据手术疗法的差异将其分成两组,即研究组（ 75 例）和对照组（ 75 例）,研究组都采用微创的保胆取石手术疗法,对照组都采用经过腹腔镜的胆囊切除手术疗法.在手术后统计比较两组病人的手术时长和生活的质量情况,分析疗效.结果 研究组病人的住院时长是（2.98±0.26）天,而对照组病人的住院时长是（6.77±0.30）天,研究组病人的住院时长明显少于对照组病人（P 〈 0.05）.在手术后,研究组病人的肛门排气时长明显优于对照组病人（P 〈 0.05）.研究组病人的不良反应发生率明显低于对照组（P 〈 0.05）.在手术结束后的半年,研究组病人的 GLOI 评分是（ 110.74 ± 3.88 ）,对照组病人的 GLOI 评分是（97.33±3.09）,研究组病人的 GLOI 评分明显高于对照组病人（P 〈 0.05）.结论 采用微创的保胆取石手术疗法对胆结石疾病的疗效显著,能够大大减少病人的住院时长,降低手术的出血量,并发症比例更低,明显改善病人的生活质量,值得临床广泛开展.

Objective To analyze and compare the cholecystolithotomy surgery and laparoscopic cholecystectomy for gallstone treatment. Methods A total of 150 cases of patients with cholelithiasis in the hospital from January 2014 to December 2015. Were grouped according to the different treatment approachs, including 75 cases in the research group and 75 cases in the control group. In the research group were treated with minimally invasive gallbladder preserving operation, the control group were treated with laparoscopic cholecystectomy. Comparison of the clinical efficacy of the two groups. Results The length of stay in the study group was significantly less than that in the control group （P 〈 0.05）. After surgery, anal exhaust time in the study group were significantly better than the control group （P 〈 0.05）. The incidence of adverse reactions in the study group was significantly lower than that in the control group （P 〈 0.05）. In the six months after the surgery, the GLOI score of in the study group is significantly higher than control group patients （P 〈 0.05）. Conclusion Minimally invasive gallbladder preserving surgery of gallstone disease curative effect is remarkable, it can greatly reduce the patient length of stay in hospital, reduce the amount of bleeding and proportion of complications, improve quality of life, and it is worthy of extensive development.
