骨皮质剥脱术联合锁定接骨板内固定术治疗四肢长骨骨不连临床疗效分析

Decortication Combined with Locking Plate Fixation in the Treatment of Long Bone Nonunion Clinical Curative Effect Analysis

目的研究四肢长骨骨不连患者行骨皮质剥脱术联合锁定接骨板内固定治疗的临床效果。方法整群选取86例该院2014年4月—2015年4月期间收治的四肢长骨骨不连患者作为研究对象,将患者随机分为两组,各43例。其中43例患者单纯采用骨皮质剥脱术进行治疗作为对照组,另43例患者在上述基础上联合锁定接骨板内固定治疗作为观察组。观察两组患者手术情况、治疗效果以及并发症情况等。结果观察组患者手术时间、出血量、愈合时间分别为(168.5±34.1)min、(339.4±43.2)m L以及(5.2±0.5)月,与对照组(162.6±33.2)min、(343.5±42.6)m L以及(6.1±0.6)月对比差异无统计学意义,P>0.05。观察组、对照组治疗优良率分别为90.70%、79.07%,观察组明显较高,P<0.05。观察组、对照组并发症发生率分别为4.65%、16.28%,观察组明显较低,P<0.05。结论对四肢长骨骨不连患者采用骨皮质剥脱术联合锁定接骨板内固定术治疗效果显著,促进患者骨折愈合,有效降低并发症,值得临床应用及推广。

Objective To study the clinical effect of Decortication combined with internal fixation of locking plate in the treatment of long limb bone nonunion. Methods Group selection 86 patients with long limb bone nonunion treated from April 2014 to April 2015 in our hospital were selected. The subjects were randomly divided into two groups, 43 cases each group. The control group just took the Decortication; the observation group adopted the combined therapy of Decortication and internal fixation of locking plate. The operation condition, clinical effect and complications for two groups were observed. Results The operation time, amount of bleeding and healing time of the observation group were（168.5±34.1）min,（339.4±43.2）m L and（5.2±0.5）months respectively, compared with the control group（162.6±33.2）min,（343.5±42.6）m L and（6.1±0.6） months, P〈0.05; the healing time for observation group was significantly shorter than control group（P0.05）; the good rate for observation group（90.70%） was significantly higher than control group（79.07%）（P〉0.05）; the incidence rate of complications for observation group（4.65%） was significantly lower than control group（16.28%）（P〈0.05）.Conclusion The combined therapy of Decortication and internal fixation of locking pl ate has an obvious effect in patients with long limb bone nonunion, shorten the healing time and reduce the incidence rate of complications. It is worthy of clinical application and promotion.
