大承气汤灌肠联合西药治疗重症胃肠功能障碍44例

Forty-Four Cases of Severe with Gastrointestinal Dysfunction Treated with Major Purgative Decoction for Enema Therapy in Combination with Western Medicine

目的:观察大承气汤灌肠联合西药治疗重症胃肠功能障碍的临床疗效。方法:选取2013年1月—2015年4月本院ICU病房中符合纳入标准的患者44例,随机分为对照组和观察组各22例,对照组采用常规西医综合治疗,观察组在对照组治疗的基础上加用中药大承气汤灌肠。治疗2个疗程后评价疗效。结果:观察组的第一次排气排便时间为(1.87±0.63)d,平均住院时间为(21.66±7.33)d,对照组第一次排气排便时间为(2.76±1.03)d,平均住院时间为(31.63±8.89),组间比较差异显著(P<0.05);观察组28天存活率为27.3%,对照组28天存活率为4.5%,组间比较差异显著(P<0.05);观察组治疗后肠功能障碍评分为(1.73±0.52),显著高于对照组的(1.25±0.44),差异具有统计学意义(P<0.05);观察组有效率为90.9%,对照组有效率为63.6%,组间比较差异具有显著性(P<0.05)。结论:大承气汤灌肠联合西药治疗重症胃肠功能障碍疗效显著,能明显改善患者胃肠功能障碍。

Objective：To observe the clinical curative effect of Major Purgative Decoction for enema therapy in combination with Westen medicine on severe with gastrointestinal dysfunction. Methods：Forty-four severe patients with gastrointestinal dysfunction who were treated in our hospital and met the inclusion criteria between Jan. 2013 and April 2015 were chosen as the research sub- jects, and randomly divided into the control group and the observation group ,with 22 cases in each group. The ones in the control group were given routine Western medicine treatment, while the ones in the observation group were added Major Purgative Decoc- tion for enema based on the treatment of the control group. Tben the clinical curative effects of the two groups were evaluated after two treatment courses. Results ：In the observation group, the breaking wind and defecation time for the first time was （ 1.87 ± 0. 63） days, and the average hospitalization time was （ 21.66 ±7.33 ） days, while those are （2.76 ± 1.03 ） days and （ 31.63± 8.89 ） days respectively in the control group ; And there was a significant difference between the two groups （P 〈 0. 05 ）. The 28- day survival rate of observation group was 27.3%, while that of the control group was 4, 5% ; And the difference between them had significance （P 〈0.05 ）. After the treatment,the observation group＇s intestinal dysfunction score was （1.73 ± 0. 52 ）, which was significantly higher than that of the control group （ 1, 25± 0. 44 ） ; the difference between them was statistically significant （ P 〈 0.05）. The effective rate of the observation group was 90.9%, while that of the control group was 63.6% ; And the differ- ence between them had significance （ P 〈 0.05 ）. Conclusion ： Major Purgative Decoction for enema therapy in combination with Western medicine has a remarkable clinical curative effect on severe patients with gastrointestinal dysfunction, can significantly improve the patients＇ gastrointestinal dysfunction symptoms.
